Clinical study of neoadjuvant therapy combined with robotic radical gastrectomy for advanced Siewert Ⅱ and Ⅲ adenocarcinoma of the esophagogastric junction
Objective:To explore the safety and feasibility of neoadjuvant therapy combined with robotic radical gastrectomy for advanced Siewert Ⅱ and Ⅲ adenocarcinoma of the esophagogastric junction.Methods:The clinical data of 15 patients with advanced Siewert type Ⅱ and Ⅲ adenocarcinoma of the esophagogastric junction who underwent neoadjuvant therapy combined with robotic radi-cal gastrectomy from Jan.2018 to Nov.2023 were retrospectively analyzed.Results:All the 15 operations were successful,no conversion to laparotomy occurred.Patients received 2-5 cycles of neoadjuvant therapy with an interval of 4(2,5)weeks between the end of treat-ment and surgery.After neoadjuvant therapy,the tumor remission rate was 73.3%,the operative time was 305(280,367)min,the intra-operative blood loss was 200(110,300)mL,the incision length was 6(5,7)cm,the total number of harvested lymph nodes was(32.8±14.5),the anal exhaust time was 3(3,4)d,and the postoperative hospitalization time was 11(9,12)d.Postoperative complications occurred in 3 cases,including one case of Clavien-Dindo grade Ⅲ.Conclusions:Preoperative neoadjuvant therapy for advanced Siewert type Ⅱ and Ⅲ adenocarcinoma of the esophagogastric junction patients can achieve better tumor remission,and combined robotic radi-cal gastrectomy is safe and feasible.
