Objective:To study the classification and risk factors of postoperative complications in patients who underwent robotic radical surgery for rectal cancer.Methods:The clinical,pathological and complication data of 932 patients,who underwent robotic radical surgery for rectal cancer between Mar.2012 and Dec.2022 were retrospectively analyzed.The complications were graded according to the Clavien-Dindo classification system,and the possible risk factors related to the postoperative complications were ana-lyzed.Results:Among the 923 patients,143 patients experienced postoperative complications,with complication incidence of 15.5%(143/923).Among them,the incidence of severe postoperative complications(grade Ⅲ or above)was 6.4%,and grade Ⅲ or above postoperative anastomotic leakage(4.2%)was the most common cause of severe postoperative complications.The results of multivariate analysis showed that age above 65 was an independent risk factor for postoperative overall and systemic complications.BMI>25 kg/m2 was an independent risk factor affecting the overall,local and severe postoperative complications.Surgeon's experience≤20 cases was an independent risk factor affecting the occurrence of local and severe postoperative complications.Conclusions:The postoperative com-plications of Da Vinci robot radical surgery for rectal cancer are mainly gradeⅡandⅢcomplications.Age above 65,BMI>25 kg/m2,and surgeon's experience≤20 cases,tumor location,presence of complications,intraoperative bleeding volume are the independent risk factors affecting the occurrence of postoperative complications.In the clinical work of carrying out robotic rectal cancer surgery,special attention should be paid to this high risk group,complications should be actively prevented and treated in order to reduce the incidence of postoperative complications.
