摘要
目的:研究经阴道自然腔道内镜手术(V-NOTES)与经脐单孔腹腔镜手术(TU-LESS)治疗输卵管妊娠的安全性与可行性.方法:回顾分析2018 年7 月25 日至 2022 年3 月25 日为输卵管妊娠患者行输卵管切除术的临床资料,根据手术方式分为V-NOTES组与TU-LESS组,每组30 例.对比分析两组手术成功率、手术时间、失血量、术后排气时间、术后疼痛、住院时间等临床指标.结果:两组均顺利完成输卵管切除术.两组手术时间、手术失血量差异无统计学意义,V-NOTES组术后肛门排气更早,疼痛评分更低,住院时间更短,差异有统计学意义(P<0.05).结论:V-NOTES与TU-LESS行输卵管切除术均安全、可行,手术效果良好,术后并发症少;V-NOTES在术后排气时间、疼痛、住院时间方面具有优势,可促进术后快速康复.
Abstract
Objective:To study the safety and feasibility of transvaginal natural orifice transluminal endoscopic surgery(V-NOTES)and transumbilical laparoendoscopic single-port surgery(TU-LESS)in the treatment of tubal pregnancy.Method:A retro-spective analysis was conducted on the clinical data of patients who underwent salpingectomy for tubal pregnancy from Jul.25,2018 to Mar.25,2022.The first 30 cases who underwent the initial stage of 2 surgical approaches were selected and divided into the V-NOTES group and the TU-LESS group according to surgical approaches.Afterward,clinical indicators,such as surgical success rate,operation time,intraoperative blood loss,postoperative exhaust time,postoperative pain,and hospital stay were compared between the 2 groups.Results:All patients in the two groups completed the salpingectomy without any changes in the surgical approach,leading to a surgical success rate of 100%in both groups.Meanwhile,no significant differences were found in operation time and intraoperative blood loss between the two groups.Additionally,the V-NOTES group had shorter postoperative exhaust time,less postoperative pain score,and shorter hospital stay than the TU-LESS group,and the differences were significant(P<0.05).Conclusions:Both V-NOTES and TU-LESS salpingectomy are safe and feasible,with good surgical effects and few postoperative complications.The V-NOTES has advantages in postoperative exhaust time,pain,and hospital stay,and is more conducive to rapid recovery after surgery.
