摘要
目的:探讨腹腔镜袖状胃切除术对重度肥胖症患者人体成分的影响.方法:纳入 2021 年 1 月至 2023 年 9 月收治的54 例BMI≥40 kg/m2 的重度肥胖症患者,平均(34.9±9.97)岁,男24 例,女30 例,体重(144.25±22.41)kg,BMI(50.21±5.98)kg/m2,均行腹腔镜袖状胃切除术.观察减重手术前后颈围、胸围、腹围、臀围及人体成分分析仪上相关指标的变化.结果:54 例患者均完成腹腔镜袖状胃切除术,无中转开腹,术后未出现明显并发症.术后半年颈围、胸围、腹围、臀围、体重、BMI、人体成分细胞内水分、细胞外水分、蛋白质、无机盐、身体细胞量、体脂肪、体脂百分比、内脏脂肪面积、骨骼肌、基础代谢率及右上肢、左上肢、躯干、右下肢、左下肢肌肉量均较术前减少,差异均有统计学意义(P＜0.001).结论:腹腔镜袖状胃切除术是安全、有效的减重手术,可明显减少体重,术后细胞内外水分、肢体肌肉、蛋白质、无机盐、骨骼肌及内脏脂肪等成分均减少.
Abstract
Objective:To explore the effects of laparoscopic sleeve gastrectomy(LSG)on body composition of severe obesity patients.Methods:From Jan.2021 to Sep.2023,54 severe obesity patients with BMI≥40 kg/m2 received LSG.The mean age was(34.9±9.97)years,there were 24 males and 30 females,with body weight of(144.25±22.41)kg and BMI of(50.21±5.98)kg/m2.The changes in neck circumference,chest circumference,abdominal circumference,hip circumference,and relevant indicators from the body composition measuring instrument before and after weight loss surgery were observed.Results:All patients completed LSG without conversion to open surgery,and there were no significant complications after surgery.Six months after surgery,the neck circumference,chest circumference,abdominal circumference,hip circumference,weight,BMI,intracellular and extracellular water content,protein,in-organic salts,body cell count of the human body,body fat,body fat percentage,visceral fat area,skeletal muscle,basal metabolic rate,the muscle mass of the right upper limb,left upper limb,trunk,right lower limb,and left lower limb significantly decreased compared to before surgery,and the differences were statistically significant.Conclusions:LSG is a safe and effective weight loss surgery,which not only significantly reduces body weight,but also reduces intracellular and extracellular water,limb muscles,protein,inorganic salts,skel-etal muscle,and visceral fat after surgery.
