The clinical efficacy analysis of air pressurization in laparoscopic repair of incarcerated hernia
Objective:To investigate the clinical efficacy of air pressurization in laparoscopic repair of abdominal wall incarce-rated hernia.Methods:The clinical data of 16 patients with abdominal wall incarcerated hernia admitted from Jun.2020 to Jun.2022 were retrospectively analyzed,including 8 cases of abdominal wall hernia(4 cases of lateral abdominal wall incisional hernia,2 cases of midline incisional hernia,2 cases of parastomal hernia)and 8 cases of umbilical hernia.Laparoscopic surgery was performed with the assistance of air pressurization method,and the relevant indexes such as operation time,intraoperative hernia size and reduction,post-operative venting time,hospital stay and complications were analyzed.Results:The reduction and repair of incarcerated hernia was successfully completed in 16 patients with the assistance of air compression method.The operation time was 85~220 min,with the ave-rage of(154.70±39.06)min,the time of reducing hernia content was(18.35±4.98)min,and the diameter of hernia ring was 3~10 cm.All the 16 patients were accompanied by small intestinal incarceration,and 5 cases of intestinal necrosis were converted to open surgery and small intestinal resection was performed;2 cases of occult defect were found during operation.Postoperative exhaustion time was 1~5 d,with a mean of(3.00±1.08)d.The postoperative pain score was(3.56±4.01),which was relieved after symptomatic treatment.The postoperative hospital stay ranged from 3 to 10 d,with a mean of(6.27±1.93)d.Follow-up time ranged from 8 to 32 months,with a median follow-up time of 24 months,and none of them suffered from complications such as incision infection,intestinal obstruction,mesh infection and hernia recurrence.Conclusions:With skillful laparoscopic hernia repair technique and strict control of surgical indication,air pressurization-assisted laparoscopic surgery in the treatment of abdominal wall incarcerated hernia is safe,feasi-ble,and worthy of promotion.
