基于教学视频的自我调节学习模式在实习医生腹腔镜模拟培训中的应用

摘要
目的:对比基于教学视频的自我调节学习模式与专业医师全程指导反馈的学习模式在无腔镜手术经验实习医生腹腔镜基本技能模拟培训中的应用效果,探索经济有效的腹腔镜基本技能模拟培训方法与模式.方法:选取2023 年6 月至2024 年5 月临床实习的77 名实习医生,采用随机数字法分为实验组(n=39)与对照组(n=38).实验组采用基于教学视频的自我调节学习模式进行培训,对照组采用专业医师指导模式进行培训.以中国腹腔镜技术考核与评价第一阶段标准为指导,完成腹腔镜模拟箱三个模块的分级专业培训练习.对比分析两组培训前后的培训效果.结果:两组研究对象经过不同教学模式的腹腔镜模拟培训后,腹腔镜下的钉板移物操作时间、剪圆圈操作时间、腔内缝合操作时间均缩短,差异有统计学意义(P<0.001).培训3 周后,两组学员最终通过率差异无统计学意义(P=0.879).实验组与对照组腹腔镜考核总分差异无统计学意义[(79.7±13.3)vs.(76.7±13.6),t=0.951,P=0.345],钉板移物操作评分高于对照组[(25.5±3.7)vs.(22.7±4.6),t=2.845,P=0.006].两组学员对课程满意度评价差异无统计学意义(P>0.05).结论:基于教学视频的自我调节学习模式在无腔镜手术经验实习医生腹腔镜基本技能模拟培训中的效果不劣于面对面的专业医师指导,尤其适于技术简单的钉板移物模块的培训.
Abstract
Objective:This study aims to compare the effectiveness of self-regulated learning based on instructional videos with the traditional learning model featuring full-time guidance and feedback from professional physicians in laparoscopic simulation training for intern physicians without prior laparoscopic experience,and to explore cost-effective methods and modes for basic laparoscopic skills simulation training.Methods:A total of 77 interns who completed clinical practice from Jun.2023 to May 2024 were selected and ran-domly assigned to an experimental group(n=39)and a control group(n=38)using random number method.The experimental group received training through a self-regulated learning mode based on instructional videos,and the control group received training under a professional physician-guided mode.The training was conducted in accordance with standard in the first stage of Chinese laparoscopic skills testing and assessment,and involved completing graded professional training exercises in three modules of the laparoscopic simula-tion box.The training effects of two groups were compared and analyzed before and after training.Results:Following the laparoscopic simulation training using different teaching modes,both groups demonstrated significant reduction in the time of performing peg transfer,circle cutting,and intracorporeal suturing under laparoscopy,with statistically significance(P<0.001).Three weeks after training,there was no statistical difference in the final passing rate between the two groups(P=0.879).There was no statistical difference in the total scores of laparoscopic examination between the experimental group and control group[(79.7±13.3)vs.(76.7±13.6),t=0.951,P=0.345].However,the peg transfer operation score in the experimental group was higher than that in the control group[(25.5±3.7)vs.(22.7±4.6),t=2.845,P=0.006].There was no statistical difference in course satisfaction ratings between the two groups(P>0.05).Conclusions:The self-regulated learning mode based on instructional videos is as effective as face-to-face professional physician guidance in basic laparoscopic skills simulation training of intern physicians without prior laparoscopic experience,and is particularly suitable for training in peg transfer module with simple technology.
