Comparison of the Application of Transtarsal Sinus Incision and Lateral L-Shaped Incision in the Treatment of Calcaneal Fractures
Objective Compare the effectiveness of the transtarsal sinus incision approach and the lateral L-shaped incision approach in the treatment of calcaneal fractures.Methods A retrospective analysis was conducted on 80 cases of calcaneal fractures treated with concurrent surgery in our hospital from January 2018 to December 2022.Among them,48 cases were treated through a tarsal sinus incision and included in the observation group.32 cases were included in the control group using the lateral L-shaped incision approach.Compare the excellent ankle hindfoot function of two groups three months after surgery,as well as the Bohler angle and Gissane angle before and one week after surgery,surgical related indicators,and incidence of surgical related complications.Results The intraoperative bleeding in the observation group was less than that in the control group,and the length of hospital stay and wound healing time were longer than those in the control group,with statistically significant differences between the groups(P<0.05).After 3 months of surgery,the excellent and good rate of ankle hind foot function in the observation group was 87.50%,which was higher than 68.75%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the two groups in terms of Gissane angle and Bohler angle of the feet before and after surgery(P>0.05).After 1 week of surgery,both Gissane angle and Bohler angle in the two groups were greater than before treatment,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 4.17%,which was lower than 21.88%in the control group,and the difference was statistically significant(P<0.05).Conclusion In the surgical treatment of Sanders type Ⅱ-Ⅲ calcaneal fractures,the tarsal sinus incision approach is more effective than the lateral L-shaped incision approach.The tarsal sinus incision approach has better ankle hindfoot function recovery,smaller surgical trauma,fewer postoperative complications,and faster postoperative recovery.
