循经取穴体外冲击波联合针灸对腰椎间盘突出症患者腰椎功能及疼痛感的影响

摘要
目的 探讨循经取穴体外冲击波(ESW)联合针灸对腰椎间盘突出症(LDH)患者的影响.方法 选择 2021年11月—2023 年5 月该院收治的 92 例LDH患者为研究对象,按随机数字表法将其分为对照组和观察组,各46例.对照组采用常规治疗,观察组在对照组基础上采用循经取穴ESW联合针灸治疗.比较两组临床疗效、腰椎功能、疼痛感.结果 观察组治疗总有效率高于对照组,差异有统计学意义(P＜0.05).治疗前,两组腰椎功能比较,差异无统计学意义(P＞0.05);治疗后,两组Oswestry功能障碍指数问卷表(ODI)、日本骨科学会评分标准(JOA)评分均较治疗前改善,且观察组ODI评分低于对照组,JOA评分高于对照组,差异有统计学意义(P＜0.05).治疗前,两组疼痛评分比较,差异无统计学意义(P＞0.05);治疗后,两组疼痛视觉模拟(VAS)评分均低于治疗前,且观察组VAS评分低于对照组,差异有统计学意义(P＜0.05).结论 循经取穴ESW联合针灸治疗LDH可有效增强疗效,改善患者腰椎功能,减轻患者疼痛感.
Abstract
Objective To explore the effect of selecting acupoints along the meridian extracorporeal shock wave(ESW)combined with acupuncture on patients with Lumbar disc herniation(LDH).Methods A total of 92 patients with LDH admitted to the hospital from November 2021 to May 2023 were selected and divided into a control group and an observation group according to the random number table method,with 46 cases in each group.The control group was treated with routine treatment.On the basis of the control group,the observation group was treated with selecting acupoints along the meridian ESW combined with acupuncture and moxibustion.The clinical efficacy,lumbar function and pain were compared between the two groups.Results The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P＜0.05).Before treatment,there was no statistically significant difference in lumbar spine function between the two groups(P＞0.05);after treatment,the Oswestry Dysfunction Index(ODI)and Japanese Orthopedic Association(JOA)scores of the two groups improved compared to before treatment,and the ODI score of the observation group was lower than that of the control group,while the JOA score was higher than that of the control group,with statistical significance(P＜0.05).Before treatment,there was no statistically significant difference in pain scores between the two groups(P＞0.05);after treatment,the Visual Analogue Scale(VAS)of pain in both groups was lower than before treatment,and the VAS score of the observation group was lower than that of the control group,with statistical significance(P＜0.05).Conclusion Selecting acupoints along the meridian ESW combined with acupuncture and moxibustion in the treatment of LDH can effectively enhance the curative effect,improve the lumbar function of patients,and reduce the pain of patients.
