经鼻导管高流量氧疗联合人免疫球蛋白对NICU重症肺炎患儿肺氧合功能及免疫学指标的影响

目的 分析经鼻导管高流量氧疗(HFNC)联合人免疫球蛋白对新生儿重症监护病房(NICU)重症肺炎患儿肺氧合功能及免疫学指标的影响.方法 选取 2020 年8 月—2023 年 8 月临沂市中心医院收治的 74 例NICU重症肺炎患儿为研究对象,按随机数字表法将其分为对照组及观察组,每组 37 例.对照组采用HFNC治疗,观察组在此基础上采用人免疫球蛋白治疗.对比两组患儿的治疗效果.结果 观察组的治疗总有效率为 94.59%,高于对照组的 78.38%,差异有统计学意义(P<0.05).观察组的退热时间、肺啰音消失时间、咳嗽缓解时间分别为(3.12±0.29)d、(4.20±0.51)d、(3.36±0.53)d,均短于对照组的(4.85±0.46)d、(5.78±0.69)d、(4.95±0.71)d,组间差异有统计学意义(P<0.05).治疗后,观察组的二氧化碳分压低于对照组,血氧分压高于对照组,组间差异有统计学意义(P<0.05).观察组的各炎症因子水平均低于对照组,各免疫学指标水平均高于对照组,组间差异有统计学意义(P<0.05).结论 HFNC联合人免疫球蛋白可减轻NICU重症肺炎患儿的症状,改善其肺氧合功能,提高免疫功能,控制炎症反应,值得临床推广应用.
Effects of High Flow Nasal Cannula Oxygen Therapy Combined with Human Immunoglobulin on Pulmonary Oxygenation Function and Immunological Indicators in Neonatorum with NICU Severe Pneumonia
Objective To analyze the effects of nasal catheter high flow oxygen therapy(HFNC)combined with human immunoglobulin on pulmonary oxygenation and immunological indexes in neonatal intensive care unit(NICU)children with severe pneumonia Methods A total of 74 children with NICU severe pneumonia admitted to Linyi Central Hospital from August 2020 to August 2023 were selected as the study objects and were divided into a control group and an observation group according to random number table method,with 37 cases in each group.The control group was treated with HFNC,and the observation group was treated with human immunoglobulin on this basis.The therapeutic effects of the two groups were compared.Results The total effective rate of the observation group was 94.59%,which was higher than 78.38%of the control group,the difference was statistically significant(P<0.05).The time of fever remission,the time of lung rale disappearance and the time of cough remission in the observation group were(3.12±0.29)d,(4.20±0.51)d and(3.36±0.53)d,respectively,which were shorter than(4.85±0.46)d,(5.78±0.69)d and(4.95±0.71)d in the control group,the differences between the groups were statistically significant(P<0.05).After treatment,the carbon dioxide partial pressure of the observation group was lower than that of the control group,and the blood oxygen partial pressure was higher than that of the control group,and the differences between the groups were statistically significant(P<0.05).The levels of inflammatory factors in the observation group were lower than those in the control group,and the levels of immunological indicators were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion HFNC combined with human immunoglobulin can alleviate the symptoms of neonatorum with NICU severe pneumonia,improve their pulmonary oxygenation function,enhance immune function and control inflammatory response,which is worthy of clinical promotion and application.
