目的 分析鼻腔冲洗联合布地奈德鼻喷雾剂辅助鼻内镜手术治疗慢性鼻窦炎伴鼻息肉(CRSwNP)患者的有效性及安全性.方法 选取2021年9月—2023年9月金乡县人民医院收治的86例CRSwNP患者为研究对象,按随机数字表法将其分为对照组与观察组,各 43 例.患者均行鼻内镜手术,对照组术后予以布地奈德鼻喷雾剂,观察组在对照组基础上加以鼻腔冲洗.对比两组患者的鼻腔通气功能、鼻黏液纤毛清除功能、炎症因子水平及不良反应发生情况.结果 术后 8 周,观察组鼻腔容积为(15.71±2.83)cm3,鼻腔最小横截面积为(0.69±0.15)cm2,均大于对照组的(13.95±2.15)cm3、(0.56±0.11)cm2,鼻腔最小横截面至前鼻孔距离为(1.34±0.10)cm,糖精清除时间为(27.34±2.15)min,均短于对照组的(1.57±0.15)cm、(30.21±2.95)min,黏液纤毛清除速度为(5.23±0.71)mm/min,快于对照组的(4.49±0.67)mm/min,黏液纤毛清除率为(61.83±8.67)%,高于对照组的(55.79±7.21)%,白细胞介素-6 水平为(5.31±0.97)pg/mL,白细胞介素-8 水平为(276.53±14.35)ng/L,肿瘤坏死因子-α水平为(1.24±0.26)ng/mL,均低于对照组的(10.25±2.01)pg/mL、(325.71±18.96)ng/L、(2.70±0.41)ng/mL,组间差异有统计学意义(P<0.05).两组不良反应发生率对比,差异无统计学意义(P>0.05).结论 鼻腔冲洗联合布地奈德鼻喷雾剂辅助鼻内镜手术可改善CRSwNP患者的鼻腔通气与鼻黏液纤毛清除功能,能够控制炎症反应,且无严重不良反应.
The Efficacy and Safety of Nasal Irrigation Combined with Budesonide Nasal Spray Assisted Endoscopic Sinus Surgery in the Treatment of Chronic Sinusitis with Nasal Polyps
Objective To investigate the efficacy and safety of nasal irrigation combined with budesonide nasal spray in the treatment of chronic sinusitis with nasal polyps(CRSwNP).Methods A total of 86 patients with CRSwNP admitted to Jinxiang County People's Hospital from September 2021 to September 2023 were selected as the study objects,and the subjects were divided into a control group and an observation group according to random number table method,with 43 cases in each group.All patients underwent nasal endoscopic surgery,the control group was given budesonide nasal spray after surgery,and the observation group was given nasal irrigation based on the control group.The nasal ventilation function,mucous cilia clearance function,inflammatory factor levels and adverse reactions were compared between the two groups.Results At 8 weeks after surgery,the nasal volume and the minimum cross-sectional area of the nasal cavity in the observation group were(15.71±2.83)cm3 and(0.69±0.15)cm2,both of which were greater than(13.95±2.15)cm3 and(0.56±0.11)cm2 in the control group,and the distance from the minimum cross section of the nasal cavity to the anterior nostril was(1.34±0.10)cm,the clearance time of saccharin was(27.34±2.15)min,which was shorter than(1.57±0.15)cm and(30.21±2.95)min of the control group,the clearance speed of mucocilia was(5.23±0.71)mm/min,which was faster than(4.49±0.67)mm/min of the control group,the clearance rate of mucocilia was(61.83±8.67)%,higher than(55.79±7.21)%of the control group,interleukin-6 was(5.31±0.97)pg/mL,interleukin-8 was(276.53±14.35)ng/L,and tumor necrosis factor-α was(1.24±0.26)ng/mL,these levels were lower than(10.25±2.01)pg/mL,(325.71±18.96)ng/L and(2.70±0.41)ng/mL in the control group,with statistical significance between the two groups(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Nasal irrigation and budesonide nasal spray assisted endoscopic sinus surgery can improve nasal ventilation and nasal mucociliary clearance in patients with CRSwNP,and can control inflammatory reaction without serious adverse reactions.
