Effect of Locking Plate Internal Fixation Via Deltoid Minimally Invasive Approach on Shoulder Joint Function and Complications in Patients with Proximal Humeral Fractures
Objective To investigate the efficacy of locking plate fixation via deltoid minimally invasive approach in patients with proximal humeral fractures.Methods A total of 86 patients with proximal humerus fracture admitted to Yantai Penglai People's Hospital from October 2021 to October 2023 were selected as the research objects,and were divided into an observation group and a control group according to random number table method,with 43 cases in each group.The control group underwent locking plate internal fixation through the traditional approach,and the observation group underwent locking plate internal fixation through the deltoid minimally invasive approach.The operative indexes,shoulder joint function,quality of life and complications were compared between the two groups.Results The intraoperative blood loss in the observation group was(159.76±18.63)mL,which was less than(203.95±23.41)mL in the control group,the operative time,hospitalization time and fracture healing time were(85.41±8.32)min,(10.05±2.09)d and(123.48±15.20)d,respectively,which was shorter than(96.75±10.42)min,(12.46±2.71)d and(133.95±17.56)d in the control group,and the differences between groups were statistically significant(P<0.05).The complication rate of observation group was 4.65%,which was lower than 18.60%of the control group,and the difference was statistically significant(P<0.05).Three months after operation,the Constant-Murley Shoulder Scale score the observation group was(77.83±4.97)points,which was higher than(68.97±4.35)points of the control group,the Generic Quality of Life Inventory-74 scores of physical function,psychological function,material life status and social function were(81.25±5.32)points,(79.94±4.88)points,(78.87±4.63)points and(84.72±5.41)points,respectively,they were higher than(73.46±4.59)points,(70.56±4.12)points,(72.98±4.15)points,(76.35±4.98)points in the control group,and the differences between groups were statistically significant(P<0.05).Conclusion Locking plate internal fixation via deltoid minimally invasive approach has the advantages of less trauma and fewer complications,which can restore shoulder joint function and improve quality of life in patients with proximal humerus fractures,and is worthy of clinical promotion and application.
