关节镜辅助双滑轮结合缝线桥技术应用于肩袖损伤患者的效果观察

目的 分析关节镜辅助双滑轮结合缝线桥技术在肩袖损伤患者中的应用效果.方法 选取 2021 年 4 月—2023年6月枣庄市立医院收治的 68 例肩袖损伤患者为研究对象,按随机数字表法将其分为对照组和观察组,各 34例.对照组行常规关节镜下缝线桥固定术,观察组行关节镜辅助双滑轮结合缝线桥技术,术后随访 3 个月.对比两组的疼痛程度、肩关节功能、肩关节活动度、生活质量、并发症发生情况.结果 术后,观察组疼痛视觉模拟疼痛评分法评分为(1.21±0.43)分,低于对照组的(2.48±0.63)分,Constant-Murley肩关节功能评定量表评分为(78.63±7.69)分,美国加州大学肩关节评分系统评分为(29.87±3.83)分,生活质量综合评定问卷各维度评分分别为(76.95±6.34)分、(77.84±5.13)分、(78.83±4.45)分、(78.96±5.47)分,均高于对照组的(65.21±5.71)分、(24.20±3.15)分、(67.21±5.75)分、(68.36±4.21)分、(69.25±3.97)分、(70.54±4.30)分,肩关节前屈、外旋活动度分别为(141.25±8.21)°、(47.21±6.73)°,均大于对照组的(130.68±6.89)°、(40.35±4.89)°,组间差异有统计学意义(P<0.05).观察组并发症发生率低于对照组,差异有统计学意义(P<0.05).结论 关节镜辅助双滑轮结合缝线桥技术可减轻肩袖损伤患者疼痛,恢复肩关节功能,有利于肩关节活动度的增大,改善生活质量,且并发症少,临床可推行应用.
Observation on the Effect of Arthroscopic Assisted Double Pulley Combined with Suture Bridge Technology in the Treatment of Patients with Rotator Cuff Injury
Objective To analyze the effect of arthroscopic assisted double pulley combined with suture bridge technology in patients with rotator cuff injury.Methods A total of 68 patients with rotator cuff injury admitted to Zaozhuang Municipal Hospital from April 2021 to June 2023 were selected as the research objects and were divided into a control group and an observation group using a random number table method,with 34 cases in each group.The control group underwent routine arthroscopic suture bridge fixation surgery,while the observation group underwent arthroscopic assisted double pulley combined with suture bridge technology,the postoperative follow-up was 3 months.Compare the pain level,shoulder joint function,shoulder joint range of motion,quality of life,and complications between two groups.Results After the surgery,the Visual Analogue Pain Scale score of the observation group was(1.21±0.43)points,lower than the control group's(2.48±0.63)points;the Constant Murley Shoulder Function Scale score was(78.63±7.69)points,and the University of California Shoulder Joint Scoring System score was(29.87±3.83)points,respectively,the scores of each dimension in the Generic Quality of Life Inventory-74 were(76.95±6.34)points,(77.84±5.13)points,(78.83±4.45)points,and(78.96±5.47)points,respectively,which were higher than the control group's(65.21±5.71)points,(24.20±3.15)points,(67.21±5.75)points,(68.36±4.21)points,(69.25±3.97)points,and(70.54±4.30)points,the shoulder flexion and external rotation range of motion were(141.25±8.21)° and(47.21±6.73)°,which were greater than(130.68±6.89)°,(40.35±4.89)°,of control group,and the differences between groups were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Arthroscopic assisted double pulley combined with suture bridge technology can alleviate pain in patients with rotator cuff injury,restore shoulder joint function,improve shoulder joint mobility,improve quality of life,and have fewer complications.It can be clinically applied.
