Management Effect of Active Health Management Model on"Three Highs"Patients in Community
Objective To explore the management effect of active health management model on"three highs"patients in community. Methods From May 2021 to May 2023,one hundred and twenty-seven patients with"three highs"who were permanent residents within the jurisdiction of community health centers of Pingshan District People's Hospital of Shenzhen City were selected as the research objects. They were divided into the control group(n=64)and the study group(n=63)by random number table method. The control group implemented routine management, while the study group implemented active health management. The weight, waist circumference, blood pressure, blood glucose, blood lipid levels, complications and compliance rate of the two groups were compared. Results After management, the weight of the study group was lower than that of the control group, and the waist circumference was smaller than that of the control group, and the differences between the groups were statistically significant(P﹤0.05);the levels of diastolic blood pressure, systolic blood pressure, fasting plasma glucose,2 hours postprandial blood glucose and glycated hemoglobin A1c in the study group were lower than those in the control group, and the differences between the groups were statistically significant(P﹤0.05);the levels of cholesterol, triglyceride and low-density protein cholesterol in the study group were lower than those in the control group, and the level of high-density protein cholesterol was higher than that in the control group, and the differences between the groups were statistically significant(P﹤0.05).The compliance rate of the study group was 98.41%,which was higher than 84.78%of the control group, and the complication rate was 0.00%,which was lower than 9.38%of the control group, and the differences between the groups were statistically significant(P﹤0.05).Conclusion Active health management model can effectively reduce the blood pressure, blood lipid and blood sugar levels of the"three highs"patients in the community, reduce complications, and improve the compliance rate of patients.
