摘要

目的:探讨18F -FDG PET/CT联合血清CA125测定对卵巢癌早期诊断的临床价值.方法:110例B超发现卵巢病变患者均行全身18F -FDG PET/CT显像及血清CA125测定.结果:在110例卵巢病变患者中,卵巢癌70例,良性病变40例,卵巢癌与卵巢良性病变患者血清CA125水平差异有统计学意义(P<0.05);卵巢癌与卵巢良性病变SUV最大值(SUVmax)差异有统计学意义(P<0.01);18F -FDG PET/CT对卵巢癌诊断的灵敏度、特异性、阳性预测值、阴性预测值、准确性分别为95.1%、79.09%、88.5%、93.1%、80.9%;CA125对卵巢癌的诊断效能均低于18F-FDG PET/CT,但两者联合可提高18F-FDG PET/CT诊断的特异性(91.0%)、准确性 (88.3%).结论:18F-FDG PET/CT对卵巢癌的诊断具有高灵敏度和相对特异性,18F -FDG PET/CT联合血清CA125测定能够提高早期诊断卵巢癌的特异性、准确性.

Abstract

Objective To discuss the early diagnostic value of ovarian cancer with 18F-FDG PET/CT and Serum CA125 determination. Methods 110 patients with lesions of the ovarian confirmed for the study. Whole body "F-FDG PET/CT scanning was performed and the level of serum CA125 was measured. Results Among 110 patients with ovarian lesion 70 patients were diagnosed as ovarian cancer and other 40 patients belong to the ovarian benign diseases. Serum CA12S level in patients with ovarian cancer was significantly higher than that of ovarian benigo disease (7><0.05). The value of SUVmax in the two groups of patients were significantly different (P<0.01). The sensitivity, specificity, positive and negative predictive value,accuracy of 18F-FDG PE7T/CT were 95.1% , 79.09% ,88.5% ,93.1% ,80.9% respectively. The diagnostic value of CA12S was lower than that of "F-FDG PET/CT, while combined application of diagnosis specificity increased to 91.0% , and accuracy increased to 88.3% individually. Conclusion 18 F-FDG PET/CT possess high sensitivity in diagnosis of ovarian malignancies and relative specificity which would increase the diagnostic specificity and accuracy combined with CA12S.
