肝豆状核变性患者肾脏纤维化相关指标的分析

摘要
目的:研究肝豆状核变性(HLD)患者血清转化生长因子β1(TGF-β1)、Ⅲ型前胶原(PCⅢ)、Ⅳ胶原(Ⅳ-C)、抗肾小球基底膜抗体(AGBM)和抗中性粒细胞胞浆抗体(ANCA)水平及其对诊断HLD患者肾脏纤维化的意义.方法:210例HLD患者和40名正常对照者,使用化学发光法检测血清PCⅢ和Ⅳ-C水平;使用ELISA检测TGF-β1、AGBM和ANCA水平.按尿常规检查是否正常,将210例HLD患者中的149例分为尿常规检查正常组(61例)和尿常规检查异常组(88例);在各组之间进行血清TGF-β1、PCⅢ和Ⅳ-C水平比较.结果:HLD患者TGF-β1、PCⅢ和Ⅳ-C水平均显著高于正常对照组;AGBM水平显著低于正常对照组;ANCA水平与正常对照组区别没有统计学意义.尿常规检查异常组与尿常规检查正常组血清TGF-β1、PCⅢ和Ⅳ-C水平差异没有统计学意义.结论::HLD患者存在肾脏纤维化,其肾脏纤维化与AGBM或ANCA相关性自身免疫性肾损害无关联.
Abstract
Objective To investigate the changes of serum transforming growth factor β1 (TGF-β1 ) , procolkgen type Ⅲ ( PC Ⅲ),Ⅳ collagen (Ⅳ-C), anti-glomerular basement membrane antibodies (AGBM) and ann-neutrophil cvtoplasmic antibody (AN-CA) levels in patients with hepatolenticular degeneration (HLD)and its significance for the diagnosis of HLD patients with renal fibro sis. Methods 210 cases of HID patients and 40 normal controls, detected serum PC Ⅲ and Ⅳ-C levels by the chemiluminescence assay, measured TGF-β1, AGBM and ANCA levels by ELISA. 149 HLD patients were divided into the notmal urine toutine examination group and the abnormalities of urine routine examination group according to finrtinga and compnued to,the levels of TGF-Β1 , PC Ⅲ and Ⅳ-C in each group. Results The levels of TGF-β1, PC Ⅲ and Ⅳ-C in HLD patients were significantly higher than the normal controls(P<0.01). AGBH level was distinctly lower than the control group; ANCA level and normal control group difference showed no significance statistically. TGF-β1, PC Ⅲ and Ⅳ-C levels manifested no significant difference in the abnormal group and normal group of routine urine examination. Conclusion HLD patients suffering from renal fibrosis. Its renal fibrosis is not associated with AGBM or ANCA-associated autoimmune kidney damage.
