甲状腺激素及TSH在老年糖尿病合并高血糖高渗状态的临床分析

摘要
目的:观察甲状腺激素及TSH在老年糖尿病合并高血糖高渗状态的变化.方法:选择2007年6月~2012年3月住院的老年糖尿病合并高血糖高渗状态的患者56例,观察其在治疗前、后甲状腺激素及TSH的变化,同时把老年糖尿病没有高渗状态的患者设立为对照组,观察其甲状腺激素及TSH的变化.结果:老年糖尿病合并高血糖高渗状态患者的TT3、TT4、FT4水平下降,患者的高渗状态纠正3周后恢复,两者有统计学差异,其FT3、TSH治疗前、后无明显变化,而老年糖尿病无高血糖高渗状态患者(对照组)的甲状腺激素无明显变化,后者与老年糖尿病合并高血糖高渗状态的患者治疗前相比,两者有统计学差异.结论:老年糖尿病合并高血糖高渗状态的患者,甲状腺激素TT3、TT4、FT4水平下降,经过治疗,患者的高渗状态纠正3周后后,TT3、TT4、FT4水平恢复,而其FT3、TSH治疗前、后无明显变化.
Abstract
Objective To observe the changes of thyroid hormones and TSH in elderly diabetics with hyperosmokr hyperglycemic state(HHS). Methods The serum concentrations of thyroid hormones and TSH both before and after treatment were measured and compared to 56 hospitalized elderly diabetic patients with HHS from June 2007 to March 2012 and the elderly patients with diabetes melhhis but not HHS served as control group. Results The levels of TT3, TT4, BT4 in elderly patients with HHS showed decreased and recovered after hyperosmolar state was corrected three weeka biter, the results indicated statistical differences. But no significant changes in the levels of FT, TSH both before and after treatment. However, the levels of thyroid hormones in the control group expressed no significant changes, combined with the elderly diabetics with HHS, the statistical differences existed. Conclusion The levels of TT3, TT4, FT4 in elderly patients with HHS decreased and recovered when the patients' hyperosmolar state were corrected after 3 weeks. There was no significant changes in the levels of FT,, TSH both before and after treatment.
