摘要

目的:检测多发性外伤患者血浆IL-11浓度,并分析其临床意义.方法:选取92例多发性外伤患者和90例健康体检正常者,ELISA检测血浆IL-11浓度,统计分析其临床价值.结果:多发性外伤患者血浆IL-11浓度(34.6±15.2)pg/ml较正常对照组(10.9±3.7)pg/ml显著升高(P<0.01).行Spearman相关分析,血浆IL-11浓度与损伤严重度评分(r=0.667,P<0.01)、急性病理生理学和长期健康评价Ⅱ评分(r=0.643,P<0.01)及中性粒细胞计数(r=0.518,P<0.01)显著正相关.经Logistic回归分析,血浆IL-11浓度(OR=1.283,95%CI=1.032~2.909,P<0.01)是多发性外伤并发多器官功能障碍综合征的独立危险因素.ROC曲线分析,血浆IL-11浓度对多发性外伤并发多器官功能障碍综合征有显著预测价值(曲线下面积=0.857,95%CI=0.786~0.920,P<0.01),且判定浓度>44.8 pg/ml为截止值时,对预测多发性外伤并发多器官功能障碍综合征有84.2%敏感度和74.1%特异度.结论:多发性外伤患者血浆IL-11浓度显著升高,血浆IL-11浓度与疾病严重程度显著相关,临床检测这个指标有助于早期判断多发性外伤并发多器官功能障碍综合征.

Abstract

Objective This study aimed to determine plasma interleukin-11 concentration and to explore its clinical significance in multiple trauma patients. Methods Ninety-two multiple trauma patients and 90 healthy controls were recruited. Concentration of interleukin-11 in plasma was measured with ELISA. Its clinical values was analyzed statistically. Results Plasma interleukin-11 concentration was markedly higher in patients than in healthy controls (34.6 ±15.2 pg/ml vs. 10.9 ±3.7 pg/ml, P<0.01) using t test, highlyl associated with injury severity seore(r=0.667,P<0.01) , acute pathophysiology and chronic health evaluation Ⅱ score(r = 0.643,P<0.01) and blood neutrophil count(r=0.518,P<0.01)using spearman's rank correlation coefficient, identified as an independent predictor for multiple organ dysfunction syndrome (MODS) secondary to multiple trauma ( OR = 1. 283, 95% (3 = 1. 032 ~ 2.909, P< 0.01) using logistic regression analysis, and predicted MODS secondary to multiple trauma obviously (Area under curve = 0.857, 95% CI =0.786~0.920, P<0.01) that chosen cutoff value of 44.8 pg/ml with the 84.2% sensitivity and 74.1% specificity using receiver operating characteristic curve analysis. Conclusion Plasma interleukin-11 concentration was increased in multiple trauma patients, plasma interleukin-11 concentration is closely correlated with disease severity, and clinical determination of such indicator might be belpful to early evaluate MODS secondary to multiple trauma.
