proGRP、NSE在小细胞肺癌鉴别诊断中的意义

摘要
目的:胃泌素释放肽前体(pro-gastrin-releasing peptide,proGRP)和神经特异性烯醇化酶(neuron specific enolase,NSE)均是小细胞肺癌的重要肿瘤标志物,不仅可用于早期筛查,对于各种良恶性肺部疾病的鉴别诊断也起到重要作用.本文拟评估两者在小细胞肺癌诊断和鉴别诊断中的价值.方法:回顾性分析298例SCLC与368例NSCLC以及肺部良性疾病中两者的水平,SPSS13.0进行统计分析,SAS 9.0进行ROC曲线绘制.结果:无论是proGRP还是NSE均在小细胞肺癌中明显升高,与其他病理类型肺癌或结核、肺炎相比均呈明显差异,proGRP的敏感性为67.1%,特异性为98.5%,NSE敏感性为60.1%,特异性为89.3%.proGRP、NSE及两者联合诊断SCLC的ROC曲线下面积分别为0.845、0.810、0.894.结论:proGRP和NSE均为SCLC诊断及鉴别诊断中非常好的标志物,两者联合诊断更加具有价值.
Abstract
Objective proGRP and NSE are very important SCLC tumor markets, they are used not only in early screening, but also in diagnosis and differential diagnosis of benign or malignant pulmonary diseases. The aim of this study is to evaluate the value of two marker in diagnosis of SCLC and differential diagnosis. Methods Retrospectively analyze proGRP and NSE serum levels in 298 cases of SCLC, 368 eases of NSCLC and some benign disease patients. All data were analyzed by SPSS 13.0 and ROC was generated by SAS 9.0. Resuits proGRP and NSE are all significantly elevated in SCLC than in other pathological type, tuberculosis and pneumonia. Sensitivity and specificity of proGBF are 67.1% and 98.5% , respectively. Sensitivity and specificity of NSE are 60.1% and 89.3% , individually. ROC area under curve of proGRP, NSE and both are 0.845, 0.810 and 0.894, separately. Conclusion proGRF and NSE are all very useful tumor marker far SCLC and combination of the two markers in diagnosis and differential diagnosis will be more valuable.
