摘要

目的:通过测定分析慢性阻塞性肺疾病(COPD)患者血清相关指标水平的变化,探讨与病情进展的关系.方法:32例急性加重期患者设置为急性加重组,30例症状处于缓解期COPD患者设置为缓解期组,30名同期的体检健康者为正常对照组.三组的血清中性粒细胞弹性蛋白酶(NE)、内脂素(Visfatin)和脂联素(APN)三项指标水平均采用酶联免疫分析法测定.测定数据进行统计处理.结果:三组之间吸烟指数差异非常显著(P<0.01),尤以急性加重组为著.三项肺功能指标即:第一秒用力呼气容积(FEV1)、FEV1%占预计值(FEV1%pred)和FEV1/ FVC%(用力最大肺活量百分)于三组之间都呈非常显著性差异(P均<0.01),且其水平的升高与病情呈平行相关.余项目各组间均无明显差异(P>0.05).急性加重组三项炎性指标水平及吸烟指数与肺功能指标FEV1%pred的相关性分析结果表明,r值分别为-0.961、-0.802、-0.624、-0.598(P<0.01,P<0.05,P<0.05),表明炎性指标水平升高及吸烟指数增多和患者的肺功能受损情况关系密切,即与病情的急性加重相关,两者呈显著负相关.而吸烟指数与三项炎性指标水平之间分别呈显著正相关(r=0.932、0.811、0.724,P<0.01、P<0.01、P<0.05).结论:三项炎性指标水平的变化在COPD的病理进程中起了重要的作用.数据表明,吸烟可能是本病发生的危险因子之一;炎性因子水平的升高对于病情的评估可能有帮助.

Abstract

Objective To investigate the clinical significance on serum related indicators in patients with COPD. Method The COPD group was divided into two groups: ① acute exacerbation chronic obstructive pulmonary disease (AE-COPD) phase group( n = 32) ;②stable chronic obstructive pulmonary disease (ST-COPD) phase group(n=30), and control group(n = 30) composed of 30 healthy persons. Then the measured data statistics and analysis. The levels of serum NE , Visfatin and APN in aboved groups were measured by ELISA. Results The statistics data of table 1 showed the levels of three pulmonary function indicators and smoking index presented significant difference between The three sets of data( all P < 0. 01) , the levels of remaining five showed no significant difference( all P > 0.05). The level of NE in AE-COPD group was notably higher than that in ST-COPD group and control group(P< 0.05, P < 0.01) , and the former is significantly higher than the latter. The statistical results of remaining two inflammatory markers basic consistent with NE. Three inflammatory markers and smoking index with FEV1% pred level indicated a significant negative correlation. However, smoking index, and three inflammatory markers manifested significantly negatively correlated. Conclusion The significantly higher of three inflammatory indicators and smoking index plays an important role in the pathogenesis of COPD. Smoking is possibly one of the risk factors of COPD, the rise of serum levels of inflammatory indicators may help to assess the pathosis.
