摘要
目的:探讨急性脑梗死(ACI)患者血清脂联素(APN)水平与颈动脉粥样硬化之间的相关性.方法:选择40例ACI患者为脑梗死组,同期健康体检正常者30例为正常对照组,检测血脂、高敏C反应蛋白(hs-CRP)和血清APN的含量,并应用彩色多普勒超声检查颈动脉粥样硬化斑块及颈动脉内膜-中膜厚度(IMT).结果:ACI组血清APN水平(中位数6.48mIU/L)明显低于正常对照组(中位数11.57mIU/L)(U=155.0,P<0.01);颈动脉IMT脑梗死组(1.19±0.35)mm高于正常对照组(1.02±0.19)mm(P<0.05);在ACI组中,血清APN水平与颈动脉IMT水平(P<0.01)和hs-CRP水平(P<0.05)呈显著负相关;多元线性逐步回归分析显示,血清APN是ACI的重要危险因子之一.结论:血清APN水平与炎性反应和颈动脉粥样硬化密切相关,是ACI强的危险预测因素.
Abstract
Objective To investigate the correlation between serum level of adiponectin and carotid artery atherosclerosis in patients with acute cerebral infarction( ACI). Methods Forty patients with ACI were assigned as experimental group and thirty healthy people served as control group. The serum level of adiponectin was measured. And the carotid plaques as well as the changes of carotid intimal-medial thickness(IMT) were examined by color dopplerultrasonography. Results The median level of adiponectin was significantly lower in ACI group(6.48mIU/L) than that of control group (11.57mIU/L) (U =155.0,P<0.01). The increased carotid DMT was higher in ACI group than that in control group [(1.19 ±0.35) mm vs(1.02 ±0.19)mm,P<0.05]. In ACI group .adiponectin level showed remarkably correlated with carotid DMT (r= - 0.409,P< 0.01) and hs-CRP(r= -0. 382,P <0.05). Multiple linear stepwise regression analysis indicated that adiponectin was one of the important risk factors in ACI. Conclusion These findings suggest the potential role of adiponectin in the development of carotid artery atherosclerosis and especially its involvement in the progress of ACI.
