99mTc-MDP全身骨扫描和血清OPG、BSP和TRACP-5b测定对肺癌骨转移的临床评价

摘要
目的:目的:探讨肺癌患者三项血清骨代谢指标含量变化与其骨转移的关系及其临床评价.方法:血清OPG、BSP和TRACP-5b测定均采用酶联免疫法.结果:测定结果表明,三项血清骨代谢指标含量骨转移组显著高于非骨转移组和正常对照组(P均<0.01),而非骨转移组与正常对照组比较无显著性差异(P均>0.05).骨转移累及数目≥3处组三项血清骨代谢指标含量显著高于≤2处组和非骨转移组(P均<0.05,P均<0.01),而≤2处组与非骨转移组差异也具显著性(P均<0.05).18例肺癌患者骨转移累及灶数统计结果,≤2处转移灶数为22个,≥3处转移灶数为120个,总体转移灶数为142个.血清骨代谢指标含量与转移灶数目的相关性分析显示,≤2处累及数目与OPG、BSP、TRACP-5b相关关系呈显著正相关(r=0.569、0.559、0.657,P均<0.05);≥3处转移灶数与OPG、TRACP-5b正相关关系更为显著(r=0.809、0.965,P均<0.01),但与BSP的关系与(1~2)处转移组相仿(r=0.617,P<0.05).结论:三项血清骨代谢指标含量变化与其骨转移的关系密切,其测定对于肺癌骨转移的诊断和监测有一定的帮助.
Abstract
Objective To explore the clinical assessment on lesions shown 99mTc-MDP whole body bone scan and determination of serum levels of OPG, BSP and TRACP-5b in lung cancer patients with bone metastasis. Methods Serum levels of OPG, BSP and TRACP-Sb (with EUSA) were determined in ①18 lung cancer definite bone metastatic lesions shown on 99mTc-MDP whole body bone scan,②20 lung cancer patients without bone metastasis and,③35 normal controls. Results The serum levels of OPG, BSP and TRACP-Sb in patients with bone metastasis showed significantly higher than in those without bone metastasis and controls ( all P < 0.01). Among them, the 3 serum marker levels in patients with ≥3 bone lesions proved notably higher than the patients with ≤2 bone lesions and without bone metastasis (the lesions were classified malignant metastasis as defined in this article) (all P <0.05, all P <0.01). There were 142 bone lesions all told in 18 lung cancer with bone metastasis. The statistics expressed metastases number were positively related with the serum levels of OPG, TRACP-5b and BSP, specially in patients with ≥3 bone lesions (r = 0. 809, 0.965,P<0.01, r = 0.617 ,P< 0.05). Conclusion Overexpressing of serum OPG, BSP and TRACP-5b closely related with osseous metastases in lung cancer, so determination of the serum markers might be helpful to diagnose and monitor the bone metastasis.
