抗凝血酶Ⅲ与紫癜性肾炎患儿肾脏受累程度的相关性分析

目的:探讨抗凝血酶Ⅲ(AT-Ⅲ)与紫癜性肾炎(HSPN)患儿肾脏受累程度的关系.方法:以86例紫癜性肾炎患儿为对象,检测其AT-Ⅲ、纤维蛋白原降解产物(FDP)、D-二聚体(D-D)、血小板(platelet,PLT)和24h尿蛋白定量(24hU-TP).根据24hU-TP结果,分为24hU-TP≥1g/24h组和24hU-TP<1g/24h组,首先单因素分析比较两组AT-Ⅲ、FDP、D-D和PLT的差别,然后,进行24hU-TP与AT-Ⅲ、FDP、D-D和PLT的相关性分析,最后,以AT-Ⅲ、FDP、D-D和PLT为自变量,以24hU-TP为应变量,进行多元回归分析,探讨AT-Ⅲ、FDP、D-D和PLT对24hU-TP的影响.结果:①24hU-TP≥1g/24h组AT-Ⅲ、FDP、D-D和PLT均显著高于24hU-TP<1g/24h组,差异均有统计学意义(均P<0.05);②24hU-TP与AT-Ⅲ、FDP、D-D均显著正相关(r分别为0.521、0.432、0.226,均P<0.05);③多元回归分析提示,AT-Ⅲ(95%CL＝0.78～0.934,P<0.05)、FDP(95%CL＝1.075～1.223,P<0.05)是24hU-TP的独立危险因子.结论:AT-Ⅲ、FDP与HSPN患儿肾脏受累程度正相关.
Abstract
Objective To explore the relationship between antithrombin Ⅲ ( AT-Ⅲ ) and the degree of renal injury in children with Henoch-Schonlein purpura nephritis(HSPN). Methods The concentrations of AT-Ⅲ, fibrinogen degradation products(FDP) , D -dimer(D-D) , platelet( PLT) and 24 hour urinary total protein(24hU-TP)were measured in 86 HSPN children. They were divided into the group A(24hU-TP≥1g/24h) and group B (24hU-TP < 1g/24h) according to the results of 24hU-TP. Firstly, the single factor a-nalysis was used to compare the concentrations of AT-Ⅲ , FDP, D-D and PLT; and then, the correlative analysis and the multiple regression analysis were used to explore the relationship of AT-Ⅲ, FDP, D-D, PLT and 24hU-TP. Results The concentrations of AT-Ⅲ, FDP, D-D and PLT in the group A proved significantly higher than that of group B(P <0.05) ; the AT-Ⅲ, FDP, D-D presented significantly positive correlate with the 24hU-TP( P <0.05). The multiple regression analysis showed that the AT-Ⅲ (95%CL =0.78-0.934,P <0.05) and the FDP(95% CL = 1.075-1.223 ,P <0.05) were independent risk factor of 24hU-TP. Conclusion The AT-Ⅲ and FDP expressed positive correlate with the degree of renal injury in children with HSPN.
