EOS及IgE对慢性鼻-鼻窦炎鼻内镜微创手术后疗效的影响

摘要
目的:通过对慢性鼻-鼻窦炎(chronic rhino-sinusitis,CRS)患者外周血嗜酸性粒细胞(eosinophil,EOS)和鼻腔病理组织中EOS、免疫球蛋白E(IgE)的检测,了解和探讨这些因素与慢性鼻-鼻窦炎鼻内镜微创手术后恢复的相关性,达到预防和降低CRS复发的可能.方法:将患者分为鼻炎组(对照组)、鼻窦炎组、鼻窦炎伴鼻息肉组.测量患者外周血EOS,并对三种病变组织制作病理切片,观察高倍镜下EOS,通过免疫组化技术计数IgE.使用Lund-Kennedy内镜黏膜形态评分系统,对三组患者进行术后评估,然后了解EOS、IgE对于CRS的影响.结果:外周血中EOS采用秩和检验(P=0.561>0.05),三组患者血中EOS没有显著性差异.组织中EOS采用秩和检验(P<0.05),三组患者组织中EOS有差别,两两比较均存在显著差异,免疫组化三组患者组织中IgE采用单因素方差分析(P<0.05),三组患者组织中IgE有差别,两两比较均存在差异,Lund-Kennedy内镜下计数采用秩和检验(P<0.05),三组患者之间存在差异.结论:通过对三组患者组织中、外周血中的EOS、组织中的IgE进行检测分析及术后Lund-Kennedy内镜下计数,说明外周血中的EOS在三组患者中的分布没有区别,因此推测外周血中EOS水平对三组患者术后恢复可能没有明显影响,但组织中的EOS、IgE对慢性鼻-鼻窦炎术后的恢复有着明显的影响.
Abstract
Objective To investigate and realize the effects of eosinophil and immunoglobulin E on patients with chronic rhinosi-nusitis, who received ESS, by testing the eosinocyte in peripheral blood and pathological tissue in nose and the immunoglobulin E in nasal tissue,that can help to prevent and decrease the relapse of CRS. Methods Divided the patients into rhinitis group( control group) ,sinusitis group,sinusitis with nasal polyp group. The eosinophil in peripheral blood and nasal mucosa were detected. Immunoglobulin E of nasal tissue were tested with immunohistochemistry, and evaluated the patients after endoscopic surgery with Lund-Kennedy endoscopic mucosal morphology scoring system. Results The data were statisticaed by SPSS 16.0 for windows. Nonparametric test showed there were no differences in eosinophil of peripheral blood for the three groups (P>0.05) , while there existe differences in eosinophil of nasal mucos between the three groups( P < 0.05 ). One-way ANOVA revealed there presented differences in immunoglobulin E among the three groups (P<0.05). And for Lund-Kennedy scores, there proved differences in the three groups (P <0.05). Conclusion Eosinophil in peripheral blood and nasal mucus, immunoglobulin E in nasal tissue and the Lund-Kennedy scores after surgery counted that eosinophil in peripheral blood distribution had no differences among the three groups, so it may have no effect on patients after the endoscopic surgery, while EOS and IgE in nasal mucosa effect patients who have received the endoscopic surgery.
