慢性牙周炎患者龈沟液EA、COL-Ⅱ和KGF-1水平的变化及其意义

摘要
目的:探讨慢性牙周炎(chronic periodontitis,CP)患者龈沟液(gingival crevicular fluid,GCF)弹性蛋白酶(elastase,EA)、Ⅱ型胶原酶(collagenase-Ⅱ,COL-Ⅱ)和角质细胞生长因子-1(keratinocyte growth factor-1,KGF-1)水平的变化及其临床意义.方法:正常对照组为58名正常成人和儿童.将62例CP患者根据不同病情分为:轻度(30例),中度(18例),重度(14例)三组.GCF中EA、COL-Ⅱ和KGF-1测定分别采用底物催化法和酶联免疫分析法.结果:EA水平轻度组与正常对照组比较略高,但无统计学差异(P>0.05),中度和重度组则显著高于正常对照组(P<0.05,P<0.01),重度组又分别显著高于轻度和中度组(P<0.05,P<0.01).COL-Ⅱ水平的变化规律同EA.KGF-1水平则轻度组与正常对照组比较呈显著高水平(P<0.05),中度和重度组水平与正常对照组比较升高更为显著(P均<0.01),重度组则显著高于轻度和中度组(P<0.05,P<0.01).结论:CP患者GCF中三项指标水平变化与病情的进展关系密切.其测定对于CP的诊断和炎症程度的评估有一定的帮助.
Abstract
Objective To explore the changes of gingival crevicular fluid EA, COL-Ⅱ and KGF-1 levels in patients with chronic periodontitis and the clinical significance of detennination. Methods According to the different degree of pathosis with 62 patients were divided to minor group(n =30) ,moderator group(n = 18) and severe group(n = 14). Their gingival crevicurlar fluid levels of COL-Ⅱand KGF-1 (with enzyme immunoassay), EA activity (with substrate catalytic method) were determined in 62 patients with chronic periodontitis and 58 healthy people served as normal control group. Results In minor group: EA level expressed slightly higher than that of controls, but without significant difference (P >0.05) , and in the moderate group and severe group:EA levels were markedly higher than those in controls(P<0.05, P<0.01) ,in the severe group EA level was obviously higher than that of minor and moderate groups (P<0.05, P<0.01). The gingival crevicurlar fluid levels of COL-Ⅱ in 3 groups with EA statistical results were consistent. The level of KGF-1 in the minor group was significantly higher than that of controls(P <0.05 ) ,the gingival crevicurlar fluid level of KGF-1 were remarkably higher in the moderate group and severe group compared with the control(all P <0.01) , and the gingival crevicurlar fluid the index level showed all apparently higher in patients of severe group than the minor and moderate groups (P < 0.05, P<0.01). Conclusion 3 indexes of gingival crevicular fluid levels changes closely related to the progress of pathosis. The de-terminations might be helpful to measure the diagnosis of CP and estimate the degree of inflammation.
