摘要
目的:探讨动态增强 MRI(DCE-MRI)定量参数鉴别诊断泪腺 IgG4 相关性病变(IgG4-RLD)、淋巴瘤、炎性假瘤的价值.方法:搜集 2010 年 9 月至 2021 年 12 月间经病理证实的泪腺 IgG4-RLD、淋巴瘤及炎性假瘤患者的病例资料;比较泪腺 IgG4-RLD组、淋巴瘤组、炎性假瘤组的 DCE-MRI定量参数(Ktrans、Kep、Ve)差异,采用 ROC 曲线分析上述参数对 IgG4-RLD 的诊断效能.结果:泪腺淋巴瘤的Ve 值(0.43±0.21)低于泪腺IgG4-RLD(0.68±0.20)和炎性假瘤(0.74±0.23),差异均有统计学意义(P 均＜0.05);泪腺 IgG4-RLD 与炎性假瘤的 Ve 值差异无统计学意义(P＝0.397).泪腺 IgG4-RLD、炎性假瘤、淋巴瘤的Ktrans、Kep差异均无统计学意义(P 均＞0.05).Ktrans、Kep、Ve 及三参数联合诊断泪腺IgG4-RLD的效能(AUC)分别为 0.545、0.540、0.545、0.620,差异均无统计学意义(P 均＞0.05).结论:Ve 有助于泪腺IgG4-RLD、淋巴瘤、炎性假瘤的鉴别;DCE-MRI 定量参数对于泪腺 IgG4-RLD的诊断有一定临床价值.
Abstract
Objective:To explore the efficiency of the quantitative parameters of Dynamic con-trast-enhanced magnetic resonance imaging(DCE-MRI)in the differential diagnosis of Immune globu-lin G4-related lacrimal disease(IgG4-RLD)from lymphoma and inflammatory pseudotumor.Methods:The data of patients with IgG4-RLD,lymphoma and inflammatory pseudotumor from September 2010 to December 2021 were acquired.The quantitative parameters of DCE-MRI including Ktrans,Kep,and Ve were compared among the three groups and the diagnostic efficacy of IgG4-RLD was predicted by ROC curve.Results:The Ve value of lymphoma(0.43±0.21)was lower than that of IgG4-RLD(0.68±0.20)and inflammatory pseudotumor(0.74±0.23),and the differences were statistically significant(P＜0.05).The Ve value was not significantly different between IgG4-RLD and inflammatory pseudotumor(P＝0.397).The Ktrans and Kep values were not significantly different among groups(P＞0.05).The ac-curacy were 0.545,0.540,0.545,and 0.620 using Ktrans,Kep,Ve,and combined there three parameters,respectively and were not significantly different(P＞0.05).Conclusions:The Ve value contributes to the differential diagnosis among IgG4-RLD,lymphoma,and inflammatory pseudotumor.The quantita-tive parameters of the DCE-MRI is value for the diagnosis of IgG4-RLD.
