目的:探讨3.0T磁共振流动敏感黑血序列三维T1加权增强扫描(FSBB 3D CE-T1WI)在脑转移瘤中的应用价值.方法:回顾性将2021年1月-2022年8月在本院就诊且经临床和影像检查证实的52例脑转移患者纳入研究.所有患者分别行颅脑FSBB 3D CE-T1WI、FSE CE-T1WI和快速梯度回波序列三维T1加权对比增强(FFE 3D CE-T1WI)扫描,采用卡方检验或Fisher精确概率检验比较3个序列之间病灶检出率的差异.结果:经随访证实颅内转移灶总数为371个,FSBB 3D CE-T1WI的检出率 99.73%(370/371),FFE 3D CE-T1WI 的检出率为 83.02%(308/371),FSE CE-T1WI 的检出率为78.44%(291/371),三者之间检出率的差异有统计学意义(x2=82.748,P<0.05).分层分析:直径<3 mm的转移灶共160个,FSE CE-T1WI、FFE 3D CE-T1WI和FSBB 3D CE-T1WI的检出率分别为50.62%、60.62%和99.37%,三个序列之间检出率的差异有统计学意义(x2=101.436,P<0.05);对于直径3~10 mm的转移灶,FSE CE-T1WI、FFE 3D CE-T1WI和FSBB 3D CE-T1WI的检出率分别为99.38%、100%和100%,三个序列之间检出率的差异无统计学意义(P>0.05);对直径>10 mm的转移灶,三个序列的检出率均为100%,差异无统计学意义(P>0.05).结论:FSBB 3D CE-T1WI在检出脑转移瘤方面要优于常规和高分辨序列MR增强扫描,值得临床推广.
Abstract
Objective:To explore the diagnostic application value of 3.0T MR 3D contrast-en-hanced T1WI flow sensitive black-blood sequence(FSBB 3D CE-T1WI)in brain metastases.Methods:Fifty-two patients with brain metastases who were admitted to our hospital from January 2021 to Au-gust 2022 and confirmed by clinical and imaging examination were retrospectively included in the study.All patients underwent brain FSBB 3D CE-T1WI,FSE CE-T1WI and fast gradient echo sequence 3D T1-weighted contrast enhancement(FFE 3D CE-T1WI)scanning,respectively.Chi-square test or Fi-sher exact probability test were used to compare the difference in lesion detection rates between the 3 sequences.Results:A total of 371 lesions were confirmed by follow-up.The detecting rate of FSBB 3D CE-T1WI,FFE 3D CE-T1WI and FSE CE-T1WI was 99.73%(370/371),83.02%(308/371)and 78.44%(291/371),respectively.Chi-square test or Fisher exact probability test showed a significant difference in detecting rate among the three groups(x2=82.748,P<0.05).Further stratified analysis showed that a total of 160 lesions with diameter<3mm were detected,of which the detection rate of FSE CE-T1WI,FFE 3D CE-T1WI and FSBB 3D CE-T1WI was 50.62%,60.62%and 99.37%,respec-tively.Chi-square test or Fisher exact probability test showed that there was a significant difference in detecting rate among the three groups(x2=101.436,P<0.05).For 3~10 mm lesions,the detection rates of FSE CE-T1WI,FFE 3D CE-T1WI and FSBB 3D CE-T1WI were 99.38%,100%and 100%,re-spectively,and there was no difference among them(P>0.05).For metastases with diameter>10mm,the detection rates of all three were 100%,with no statistical difference(P>0.05).Conclu-sion:FSBB 3D CE-T1WI sequence is superior to conventional sequence CE-MRI and high-resolution CE-MRI in detection of brain metastases,and is worthy of clinical application.
