摘要
目的:探讨合并缺血性脑卒中颈动脉蹼(CaW)的CT形态学特征.方法:回顾性收集2017年1月-2022年7月经CT血管成像(CTA)诊断的40例CaW患者的临床及影像资料,其中男32例,女8例,年龄39~80岁.根据CaW的侧别与急性脑缺血灶的关系,分为缺血性脑卒中组与非缺血性脑卒中组.比较2组CaW患者的年龄、性别以及合并高血压、冠心病、高血脂、吸烟的发生率差异有无统计学意义;比较2组CaW的形态学特征如长度、宽度、动脉蹼与血管壁的夹角、血管狭窄程度等差异有无统计学意义.结果:40例CaW患者中缺血性脑卒中19例,非缺血性脑卒中21例.缺血性脑卒中组与非缺血性脑卒中组患者的年龄、性别、合并高血压、冠心病、高血脂、吸烟的发生率差异均无统计学意义(P值均>0.05).缺血性脑卒中组的CaW长度为3.8 mm(3.5,4.5),动脉蹼与血管壁的夹角为51.2°(38.5°,59.8°),大于非缺血性脑卒中组的3.2 mm(2.4,4.1)及42.7°(34.8°,52.5°),差异均有统计学意义(Z=0.042、0.038,P均<0.05);两组间动脉蹼的宽度及造成的狭窄程度差异均无统计学意义(Z=0.946、0.085,P均>0.05).结论:CTA对CaW具有重要诊断价值,缺血性脑卒中患者CaW的长度及动脉蹼与血管壁的夹角大于非缺血性脑卒中患者.
Abstract
Objective:To analyze CT morphological feature of carotid web(CaW)in patients with ischemic stroke.Methods:The clinical and imaging data of 40 patients with CaW diagnosed by CT angiography(CTA)from January 2017 to July 2022 were retrospectively collected,including 32 males and 8 females,aged 39'80 years.Based on the relationship between the side of CaW and acute cerebral ischemia,the patients were divided into ischemic stroke group and non-ischemic stroke group.The differences in age,gender,and the incidence of comorbidities such as hypertension,coronary heart dis-ease,hyperlipidemia,and smoking were compared between the two groups.Additionally,the morpho-logical characteristics of CaW,including length,width,angle with the vascular wall,and degree of ste-nosis,were compared between the two groups.Results:There were 19 cases in the ischemic stroke group and 21 cases in the non-ischemic stroke group.There were no significant differences between the two groups in terms of age,gender,incidence of hypertension,coronary heart disease,hyperlipidemia and smoking(all P values>0.05).The length of CaW in the ischemic stroke group was 3.8mm(3.5,4.5),the angle with the vascular wall was 51.2°(38.5°,59.8°),both of which were greater than those in the non-ischemic stroke group,which were 3.2mm(2.4,4.1)and 42.7°(34.8°,52.5°)respectively,with statistically significant differences(Z=0.042,0.038;all P values<0.05);However,there were no sig-nificant differences in the width of carotid web and degree of stenosis between the two groups(Z=0.946,0.085;all P values>0.05).Conclusion:CTA played an important role in diagnosis CaW,the length of CaW and angle between web and vascular wall were greater compared to those patients with non-ischemic stroke.
