高频超声与脂肪肝指数联合评估在健康体检中诊断脂肪肝的价值

目的:探讨高频超声测定的肝脾指数与脂肪肝指数(fatty liver index,FLI)联合应用在健康体检中对代谢相关脂肪性肝病(metabolic dysfunction-associated fatty liver disease,MAFLD)的诊断效能.方法:选取2022年1月至2024年3月空军军医大学第一附属医院综合诊疗体检二部健康体检的代谢相关脂肪性肝病患者80例为MAFLD组,另选取同期健康体检人员80例为对照组.收集入选对象临床资料,采用高频超声检查测定肝脾指数,并计算FLI.通过Logistic回归分析诊断MAFLD的影响因素,受试者操作特征(receiver operating characteristic,ROC)曲线分析高频超声测定肝脾指数、FLI对MAFLD的诊断效能.结果:与对照组相比,脂肪肝组在谷草转氨酶(aspartate aminotransferase,AST)、甘油三酯(triglycerides,TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、高频超声测定肝脾指数、FLI方面差异有统计学意义(P<0.05).Logistic回归分析显示,高频超声测定肝脾指数(OR=1.415,95%CI=1.224~1.635,P<0.05)与 FLI(OR=1.060,95%CI=1.009~1.114,P<0.05)为诊断 MAFLD 的影响因素.ROC曲线分析显示,高频超声测定肝脾指数(曲线下面积(area under the curve,AUC)=0.870)、FLI(AUC=0.892)对MAFLD诊断效能良好,而联合检测时诊断AUC达0.950.结论:高频超声测定的肝脾指数与FLI联合应用可显著提高MAFLD的诊断效能,可为健康体检中的早期筛查提供有力工具.
The Diagnostic Efficacy of High-frequency Ultrasound Combined with Fatty Liver Index(FLI)for Metabolic Dysfunction-associated Fatty Liver Disease(MAFLD)in Health Examinations
Objective:To explore the diagnostic efficacy of combining liver-spleen index measured by high-frequency ultrasound with fatty liver index(FLI)for metabolic dysfunction-associated fatty liver disease(MAFLD)in health examinations.Methods:A total of 80 MAFLD patients undergoing health examinations at the Comprehensive Diagnostic and Treatment Health Examination Department Ⅱ of the First Affiliated Hospital of Air Force Medical University from January 2022 to March 2024 were selected as the MAFLD group.Another 80 individuals undergoing health examinations during the same period were selected as the control group.Clinical data of the subjects were collected,liver-spleen index was measured using high-frequency ultrasound,and FLI was calculated.Logistic regression analysis was used to analyze factors influencing MAFLD diagnosis.Receiver operating characteristic(ROC)curve analysis was performed to evaluate the diagnostic efficacy of liver-spleen index measured by high-frequency ultrasound and FLI for MAFLD.Results:Compared with the control group,the MAFLD group showed statistically significant differences in aspartate aminotransferase(AST),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C),liver-spleen index measured by high-frequency ultrasound,and FLI(P<0.05).Logistic regression analysis showed that liver-spleen index measured by high-frequency ultrasound(OR=1.415,95%CI=1.224-1.635,P<0.05)and FLI(OR=1.060,95%CI=1.009-1.114,P<0.05)were influencing factors for MAFLD diagnosis.ROC curve analysis showed good diagnostic efficacy for MAFLD with liver-spleen index measured by high-frequency ultrasound(area under the curve(AUC)=0.870)and FLI(AUC=0.893),while the combined detection achieved an AUC of 0.950.Conclusion:The combination of liver-spleen index measured by high-frequency ultrasound and FLI can significantly improve the diagnostic efficacy for MAFLD,providing a powerful tool for early screening in health examinations.
