摘要
目的:观察黄芪猪蹄膏治疗重症患者胃排空障碍的临床疗效.方法:将兰州大学第二医院ICU收治的需要早期肠内营养支持的324例患者分为4组:甲氧氯普胺组76例、红霉素组8 3例、甲氧氯普胺联合红霉素组84例;黄芪猪蹄膏组81例.4组患者均在收住ICU 24小时后的第一个8点钟给药,记录每日胃液潴留量及胃肠喂养情况.结果:1)第1、2、3、4、5、6、7天联合治疗组的胃液潴留量明显少于甲氧氯普胺组和红霉素组(P＜0.05),第3、4、5、6、7天黄芪猪蹄膏组的胃液潴留量明显少于甲氧氯普胺组和红霉素组(P＜0.05),但与联合治疗组比较无显著差异(P＞0.05);2)联合治疗组喂养成功率在3天后明显高于甲氧氯普胺组和红霉素组(P＜0.05),黄芪猪蹄膏组喂养成功率在4天后明显高于甲氧氯普胺组和红霉素组(P＜0.01),但与联合治疗组比较无显著差异(P＞ 0.05).结论:黄芪猪蹄膏能改善重症患者胃排空障碍,与联合治疗组疗效相当,其可能的机制是减少胃液潴留量,提高喂养成功率.
Abstract
Objective:To observe clinical effects of HuangQi ZhuTi ointment in treating severe patients with delayed gastric emptying.Methods:All 324 patients admitted to ICU of Lanzhou University the second hospital were divided into four groups:metoclopramide group(76 patients);erythromyein group (83 cases);metoclopramide (10 mg) combined with erythromycin (0.25g) group (84 cases);HuangQi ZhuTi ointment (81 cases).Daily gastric retention and gastrointestinal feeding were recorded after the patients in four groups were medicated in the first 8am when they were admitted to ICU in 24 hours.Results:1) Combination therapy group was less than metoelopramide group and erythromycin group obviously in gastric retention at the first,second,third,forth,fifth,sixth and seventh day (P＜0.05),HuangQi ZhuTi ointment group was less than metoclopramide group and erythromycin group in gastric retention significantly at the third,forth,fifth,sixth and seventh day (P＜0.05),but there was no significant difference compared with combination therapy group (P＞0.05);2) combination therapy group was higher than metoclopramide group and erythromycin group in feeding success rate in three days obviously (P＜0.05),HuangQi ZhuTi ointment was higher than metoclopramide group and erythromycin group in feeding success rate obviously in four days (P＜0.01),but there was no significant difference compared with combination therapy group (P＞0.05).Conclusion:HuangQi ZhuTi ointment could improve delayed gastric emptying of severe patients,equivalent to clinical effects of combination therapy group,its mechanism might be reducing gastric retention and improving feeding success rate.
