目的:通过数据挖掘对中医治疗心力衰竭医案进行集外关联解析,对心力衰竭的症状、病机与药物之间的核心关联规则进行挖掘,从中分析中医辨证治疗心力衰竭的经验.方法:在中国知识资源总库、维普中文科技期刊全文数据库、万方数据学术论文总库中,检索中医治疗心力衰竭医案类论文,采用Medcase Ver 5.2诊籍数据处理平台,基于频繁模式增长加强关联分析算法,对心力衰竭临床医案进行集外关联解析.结果:共纳入医案406则,患者406人次,971诊次,涉及病机56条,症状123种,药物348种.经集外关联挖掘,产生临床症状与药物集外关联规则31条,病机与药物集外关联规则30条,临床症状与病机集外关联规则31条.结论:中医临床辨治心力衰竭的核心症状为水肿、胸闷、乏力、心悸、纳差、气短、尿少;核心病机中脏腑病位重点为心阳虚、肾阳虚;高关联选药为茯苓、黄芪、丹参、白术、附子等,可采用温心阳、壮肾阳、利水消肿的方法,治疗心阳虚、肾阳虚等较为常见的心力衰竭.
Out-of-set Correlation Analysis of Clinical Cases of Heart Failure based on Data Mining
Objective:To conduct out-of-set correlation analysis of TCM therapy for clinical cases of heart failure via data mining,and to unearth the core association rules between heart failure symptoms,pathogenesis and drugs,and to analyze the experience of Chinese medicine in differentiating and treating heart failure.Methods:The papers of TCM treatment of heart failure were searched in CNKI,VIP and Wanfang Data,Medcase Ver 5.2 diagnostic data processing platform was applied,based on the frequent pattern growth to streng-then the association analysis algorithm,to analyze the out-of-set association analysis of clinical cases of heart failure.Results:A total of 406 medical cases were included,with 406 patients and 971 consultations,involving 56 pathogenesis,123 symptoms and 348 drugs.Via out-of-set association mining,31 out-of-set association rules between clinical symptoms and drugs,30 out-of-set association rules between pathogenesis and drugs,and 31 out-of-set association rules between clinical symptoms and pathogenesis were generated.Conclusion:The core symptoms of heart failure in Chinese medicine are edema,chest tightness,fatigue,palpitation,poor appetite,shortness of breath,and low urine output;the focus of the viscera in the core pathogenesis is heart Yang deficiency and kidney Yang deficiency;and the highly correlated drugs are Fuling(poria),Huangqi(Astragali radix),Danshen(Salviae miltiorrhizae radix et rhizoma),Baizhu(Atractylodis Macrocephalae rhizoma),and Fuzi(Aconiti lateralis radix praeparata),which can be used to warm the heart Yang,strengthen the kidney Yang,and promote diuresis and eliminate edema to treat the heart failure of heart Yang and kidney Yang deficiencies.
