穴位按压对老年转移性结直肠癌维持治疗患者癌因性疲乏的干预效果

摘要
目的:观察穴位按压对老年转移性结直肠癌维持治疗患者癌因性疲乏(cancer related fa-tigue,CRF)的干预效果.方法:前瞻性收集98例存在CRF的老年(≥65岁)转移性结直肠癌维持治疗患者,参照随机数字表法随机分为观察组50例和对照组48例.对照组仅接受常规健康指导,观察组在此基础上给予穴位按压,双侧取穴合谷、三阴交、足三里,按压手法经研究者培训指导后,由患者自行操作,每次每侧每个穴位持续按压5 min,共30 min,每天3次.两组均连续干预6周.干预前后,采用Piper疲乏修订量表和匹兹堡睡眠质量指数量表评估患者疲乏和睡眠质量变化情况,检测外周血T淋巴细胞亚群指标.结果:干预后,观察组Piper疲乏修订量表和匹兹堡睡眠质量指数量表评分低于对照组,观察组外周血CD3+、CD3+CD4+和CD4+/CD8+水平高于对照组,差异有统计学意义(P＜0.05).结论:穴位按压有助于减轻老年转移性结直肠癌维持治疗患者CRF,提高睡眠质量,可能与改善患者自身免疫功能有关.
Abstract
Objective:To observe the intervention effects of acupressure on cancer-related fatigue in elderly patients with metastatic colorectal cancer undergoing maintenance treatment.Methods:All 98 elderly(≥65 years)metastatic colorectal cancer patients suffering from CRF and accepting maintenance treatment were prospectively enrolled,and randomized into 50 cases of the observation group and 48 cases of the control group using random number table method.The control group accepted routine health guidance,and the observation group was given acupressure on the foundation of the control group,bilaterally selecting the acupoints of Hegu(LI4),Sanyinjiao(SP6)and Zusanli(ST36),pressure techniques were operated bilaterally by the patients themselves after being trained and instructed by the researcher,continuous pressure on each point was conducted on each side for five minutes at a time,altogether 30 minutes,three times each day.Both groups were intervened for six weeks consecutively.The Revised Piper Fatigue Scale(PFS-R)and the Pittsburgh Sleep Quality Index(PSQI)were adopted before and after the intervention to evaluate the fatigue and quality of sleep,to detect the indicators of peripheral blood T-lymphocyte subsets.Results:After the intervention,the scores of PFS-R and PSQI of the observation group were lower than these of the control group,the levels of CD3+,CD3+CD4+and CD4+/CD8+of the observation group were higher than these of the control group,and the difference showed statistical meaning(P<0.05).Conclusion:Acupressure could alleviate CRF in elderly patients with metastatic colorectal cancer undergoing maintenance treatment and improve the quality of sleep,which might be related to the improvement of autoimmune function.
