Influence of Xiaoqinglong Tang Combined with Western Medicine on Clinical Effects of Patients with AECOPD of External Cold and Internal Drink Pattern and Oxidative Stress

目的:探讨小青龙汤联合布地奈德混悬液雾化吸入对外寒内饮型慢性阻塞性肺疾病急性加重期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者肺功能和氧化应激水平的影响.方法:将86例外寒内饮型AECOPD患者随机分为观察组和对照组各43例.两组均予对症基础治疗,对照组予布地奈德混悬液雾化吸入治疗,观察组予小青龙汤联合布地奈德混悬液雾化吸入治疗,两组均治疗4周.比较两组治疗前后慢性阻塞性肺疾病患者自我评估测试(COPD assessment test,CAT)评分、肺功能相关指标[呼气峰流速(peak expiratory flow rate,PEF)、第一秒用力呼气量(forced expiratory volume in one second,FEV1)、第一秒用力肺活量/潮气量比值(first second forced vital capacity/tidal volume,FEV1/FVC)]、血清核因子E2相关因子2(nuclear factor erythroid 2-related factor 2,Nrf2)、活性氧(reactive oxygen species,ROS)、超氧歧化酶(superoxide dismutase,SOD)水平和中性粒/淋巴细胞比(neutrophil to lymphocyte ratio,NLR)、血小板/淋巴细胞比(platelet to lymphocyte ratio,PLR)的变化情况及临床疗效.结果:与治疗前比较,两组治疗后CAT评分均降低(P<0.05),肺功能相关指标增加(P<0.05),血清Nrf2、SOD水平升高(P<0.05),ROS水平及NLR、PLR值均降低(P<0.05);治疗后观察组上述各指标改善程度均大于对照组(P<0.05);总有效率观察组[90.47%(38/42)]高于对照组[73.81%(31/42)](P<0.05).结论:小青龙汤联合布地奈德混悬液雾化吸入治疗能有效改善外寒内饮型AECOPD患者肺功能,降低中性粒细胞比例,减轻氧化应激反应,提高临床疗效.
Objective:To discuss the influence of Xiaoqinglong Tang joined with nebulized budesonide inhalation suspension on pulmonary function and the levels of oxidative stress in patients suffering acute exacerbation of chronic obstructive pulmonary disease(AECOPD)of exterior cold and interior drink pattern.Methods:All 86 patients were randomized into the observation group and the control group with 43 cases in each group.Both groups accepted symptomatic basic treatment,the control group was given budesonide suspension for nebulized inhalation treatment,and the observation group was treated by Xiaoqinglong Tang and nebulized budesonide inhalation suspension,both groups were treated for four weeks.To compare the changes of CAT scores,pulmonary function related indexes including PEF,FEV1,FEV1/FVC,the levels of Nrf2,ROS,SOD and NLR,PLR,and clinical effects of the two groups before and after the treatment.Results:Compared with before treating,CAT scores of both groups were lowered after treating(P<0.05),pulmonary function related indexes were increased(P<0.05),the levels of Nrf2 and SOD were raised(P<0.05),the levels of ROS,NLR and PLR were reduced(P<0.05);after the treatment,the above indexes of the observation group were greater than these of the control group(P<0.05);total effective rate of the observation group was[90.47% (38/42)],higher than[73.81% (31/42)]of the control group(P<0.05).Conclusion:Xiaoqinglong Tang joined with nebulized budesonide inhalation suspension could effectively improve pulmonary function,reduce the proportion of the neutrophils,relieve oxidative stress and improve clinical effects.
