"三调针法"治疗脊髓损伤后神经源性膀胱临床观察

Clinical Observation on the Treatment of Neurogenic Bladder after Spinal Cord Injury by"Three Modulation Acupuncture"

目的:观察"三调针法"治疗脊髓损伤后神经源性膀胱(neurogenic bladder,NB)的临床疗效.方法:选择脊髓损伤后NB患者60例,随机分为对照组和观察组,每组30例.对照组采用间歇无菌导尿、训练辅助排尿等常规治疗,观察组在对照组基础上采用"三调针法"进行治疗,比较两组治疗前后尿流动力学、残余尿量的变化情况及临床疗效.结果:治疗后两组尿流动力学指标均明显改善(P<0.05),观察组较对照组改善明显(P<0.05).治疗后两组膀胱残余尿量均较治疗减少(P<0.05);观察组较对照减少明显(P<0.05);总有效率观察组[100.00%(30/30)]高于对照组[83.33%(25/30)](P<0.05).结论:"三调针法"治疗脊髓损伤后NB,可改善尿流动力学,恢复排尿功能,提高临床疗效.
Objective:To observe the curative effects of"three modulation acupuncture"in the treatment of neurogenic bladder after spinal cord injury.Methods:Sixty patients were chosen and randomized into the control group and the observation group with 30 cases in each group.The control group adopted conventional therapy including intermittent sterile catheterization,and training assisted urination,the observation group was treated with"three modulation acupuncture"based on the therapy that the control group received,to compare urodynamics and residual urine output before and after the treatment,and clinical effects between both groups.Results:After the treatment,urodynamic indexes improved noticeably in both groups(P<0.05),the improvement of the observation group was more remarkable than that of the control group(P<0.05).After the treatment,residual urine output decreased than before treating(P<0.05);the decrease of the observation group was more remarkable(P<0.05).Total effective rate of the observation group was[100.00% (30/30)],higher than[83.33% (25/30)]of the control group(P<0.05).Conclu-sion:"Three modulation acupuncture"in the treatment of neurogenic bladder(NB)after spinal cord injury(SCI)could improve urodynamics,restore urinary function and raise the therapeutic effects.
