Intervention Effects of Herbal Acupoint Application and Abdominal Massage on the Constipation after Cerebral Hemorrhage

目的:观察中药穴位贴敷联合腹部推拿对脑出血(cerebral hemorrhage sequelae,CHS)后便秘的疗效.方法:选取CHS后便秘患者80例,随机分为对照组和观察组各40例.两组均予CHS后遗症期常规干预措施,对照组予开塞露干预,观察组予中药穴贴敷联合腹部推拿,两组均干预4周.比较两组干预前后便秘相关症状评分、便秘患者症状自评量表(constipation symptoms self evaluation scale,PAC-SYM)评分、国家神经功能缺损量表(national institute of health stroke scale,NIHSS)评分、血清胃动素和胃泌素水平,观察两组临床疗效.结果:干预后两组便秘主要症状评分、PAC-SYM评分、NIHSS评分均明显降低,且观察组降低幅度大于对照组(P＜0.05);两组血清胃动素和胃泌素水平均明显升高,且观察组升高幅度大于对照组(P＜0.05);观察组总有效率[92.50%(37/40)]高于对照组[72.50%(29/40)](P＜0.05).结论:中药穴位贴敷加腹部推拿治疗CHS后便秘疗效明显,有利于减轻便秘相关症状与神经功能缺损,可促进胃肠激素的分泌.
Objective:To observe clinical effects of herbal acupoint application and abdominal massage in the treatment of cerebral hemorrhage sequelae(CHS).Methods:All 80 patients were selected and randomized into the control group and the observation group with 40 cases in each group.Conventional interventions for CHS were given to both groups,the control group accepted suppositories glycerol,and the observation group received herbal acupoint application and abdominal massage,both groups were intervened for four weeks.To compare constipa-tion-related symptom scores,constipation symptoms self evaluation scale(PAC-SYM)scores,national institute of health stroke scale(NIHSS)scores,the levels of serum motilin and gastrin in the two groups before and after the intervention,and to observe clinical effects of the two groups.Results:After the intervention,main symptom scores of constipation,PAC-SYM scores and NIHSS scores of both groups were lowered obviously,and the decrease of the observation group was greater than that of the control group(P<0.05);the levels of serum motilin and gastrin were increased remarkably,the increase of the observation group was greater than that of the control group(P<0.05);total effective rate of the observation group was[92.50% (37/40)],higher than[72.50% (29/40)]of the control group(P<0.05).Conclusion:Herbal acupoint application combined with abdominal massage in the treatment of CHS could help alleviate constipation-related symptoms and neurologic deficit,and promote the secretion of gastrointestinal hormone with noticeable clinical effects.
