Cases Report of Digoxin Combined With Amiodarone in the Treatment of Fetal Tachycardia
OBJECTIVE To study the efficacy and safety of digoxin combined with amiodarone in the treatment of fetal tachycardia.METHODS The results of ultrasound examination in the fetal Medical Department of Guangzhou Women's and children's center in the past three years were retrospectively analyzed.Fetal echocardiography and M-mode ultrasound were used to determine whether it was fetal tachycardia and determine the type of tachycardia.Patients with fetal tachycardia were treated with amiodarone hydrochloride tablets and digoxin tablets,and maternal complications were actively treated.RESULTS Seven pregnant women were diagnosed with fetal tachycardia,one of them had severe fetal edema and was transferred to the other hospital after ineffective drug treatment.During the treatment period,one case had poor medication adherence and only half of the time the fetal heart rate was in a normal state.,while the rest 5 patients had good treatment results,with no thyroid dysfunction,no QT interval extension,and no symptoms of liver damage despite abnormal liver function indicators.The cure rate was 71.4%(5/7).Among the 6 infants delivered by the aforementioned patients in our hospital,3 of them developed arrhythmia and were cured after medication treatment.4 of 6 had a tendency towards hypothyroidism,two of which recovered after taking Levothyroxine Sodium,versus the rest regained their health without medication.CONCLUSION Digoxin combined with amiodarone can effectively control fetal heart rhythm towards the patients whose tachycardia occur in fetuses with or without hydrops.In tachycardia fetuses with hydrops,it is difficult to control the heart rate with digoxin alone,and amiodarone should be added for treatment.Individualized administration according to blood drug concentration and positive symptomatic treatment can have a good prognosis.
