摘要

目的 建立高效液相色谱法(HPLC)测定隐球菌脑膜炎合并艾滋病患者血清中氟康唑浓度.方法 血清经乙酸乙酯蛋白沉淀,以非那西丁为内标,采用高效液相色谱法测定氟康唑浓度.Aglient Eclipse XDB C18为色谱柱,乙腈-水(25∶75,V∶V)为流动相,流速为0.8 mL·min-1,柱温为25 ℃,检测波长260 nm.该方法应用于5例使用高剂量的氟康唑患者血药浓度测定.结果 氟康唑测定线性范围为10~300μg·mL-1(r=0.999),定量下限为10 μg·mL-1,日内、日间RSD均<5%,提取回收率64.42%~69.77%,且该方法稳定性良好.10例患者氟康唑的AUC0-24为785~2 266 µg·h·mL-1,均能达到靶值100.结论 本研究成功建立人血清测定氟康唑的HPLC方法,适用于氟康唑浓度监测.

Abstract

OBJECTIVE To establish a high performance liquid chromatography(HPLC)method for determination fluconazole serum concentration in patients with cryptococcal meningitis and AIDS.METHODS The serum was precipitated with ethyl acetate,using phenacetin as the internal standard,and the concentration of fluconazole was determined by HPLC.The determination was performed on Aglient Eclipse XDB C18 column with mobile phase acetonitrile-water(25∶75,V∶V)at the flow rate 0.8 mL·min-1.The column temperature was 25 ℃ and the detection wavelength was 260 nm.This method was applied to the determination of blood drug concentration in 5 patients receiving high dose of fluconazole.RESULTS The linear range of fluconazole was 10-300 μg·mL-1(r=0.999),with a quantification limit of 10 μg·mL-1.RSD of intra-day and inter-day were less than 5%.Average extraction recoveries were 64.42%-69.77%,and the method has good stability.AUC24 of fluconazole in 10 patients was 785-2 266 μg·h·mL-1 and all patients reached the target value of 100.CONCLUSION This study has successfully established a HPLC method for the determination of fluconazole in human serum,which is suitable for monitoring the concentration of fluconazole.
