Analysis of the Characteristics and Rational Use of Combined Antischizophrenic Drugs in A Psychiatric Hospital
OBJECTIVE Analyzed and evaluated the characteristics and rationality of combined use of multiple psychiatric drugs in the treatment of schizophrenia in a psychiatric hospital.METHODS 499 prescriptions for discharged patients with schizophrenia issued by a tertiary psychiatric hospital in Guangzhou from June 2020 to May 2023 were retrospectively analyzed,and the combined use of all kinds of psychotropic drugs,frequency of drug use(DDDs),use of high-frequency psychiatric drugs and unreasonable drug use were analyzed.RESULTS The common combination regimens of psychiatric drugs for the treatment of schizophrenia include two-drug combination,three-drug combination and four-drug.The common types were atypical antipsychotics,mood stabilizers,anti-anxiety drugs,etc.Among them,there were 495 atypical antipsychotic prescriptions,accounting for the highest proportion of 99.20%.In the treatment of antischizophrenia,atypical antipsychotics had the highest DDDs,followed by mood stabilizers.The top 5 DDDs were risperidone,olanzapine,clozapine,valproic acid and paroperazone,respectively,and the drug utilization index of clozapine,valproic acid,lorazepam,risperidone,lithium carbonate,aripiprazole,paliperidone and zolpidem were all lower than 1.The unreasonable situation of combined drug use mainly includes the use of drugs in excess of drug instructions and the combination of drugs with the same action mechanism.There were 4 unreasonable prescriptions,and the unreasonable incidence rate was 0.80%.CONCLUSION Two combinations of atypical antipsychotic drugs combined with mood stabilizers or anti-anxiety drugs can be used in the treatment of schizophrenia.Triple or quadruple drugs should be avoided,and the management of drug use for children and teenagers under 18 years old should be strengthened.
