Association of Sirt6 and its related inflammatory factors with severity of disease in patients with septic acute kidney injury
Objective To investigate the correlation between silencing information regulatory factor 6(Sirt6)and its related inflammatory factors and the severity of sepsis acute kidney injury(SAKI).Methods A total of 172 patients with sepsis from the Second Affiliated Hospital of Guangdong Medical University from January 2021 to February 2023 were selected and divided into the combined AKI group and the non-AKI group according to whether the patients had concurrent AKI.The levels of APACHE Ⅱ score,Sirt6,TNF-α,NF-κB,IL-6,IL-18 and IL-1β were compared between the two groups,and the relationship between the levels of Sirt6 and its related inflammatory factors and the severity of patients with different diseases was analyzed.Results The levels of Sirt6 in patients with AKI were lower than those in non-AKI group,and the levels of APACHE Ⅱ score,TNF-α,NF-κB,IL-6,IL-18 and IL-1β in patients with AKI were higher than those in non-AKI group(P<0.05).In the low-risk group,TNF-α(OR=7.608),NF-κB(OR=9.840)and IL-6(OR=8.493)were the risk factors in the medium-risk group(P<0.05).Taking the low-risk group as reference,AKI stage(OR=22.427),TNF-α(OR=11.261),NF-κB(OR= 28.983),IL-6(OR=18.713),IL-18(OR=41.536)and IL-1β(OR=13.303)were the risk factors for high risk of the disease.Sirt6(OR=0.063)was a protective factor(P<0.05).ROC curve analysis showed that the AUC predicted by Sirt6,TNF-α,NF-κB,IL-6,IL-18 and IL-1β was 0.863,the sensitivity was 91.40%and the specificity was 94.90%,which was superior to each single index(P<0.05).Conclusion The expression of Sirt6 and its related inflammatory factors is obviously abnormal in the serum of SAKI patients,which is closely related to the severity of the patients'disease,and can be used as a clinical indicator to jointly evaluate the patients'disease.
