The effect and safety of esmketamine on postoperative depression,anxiety and pain in patients undergoing laparoscopic ectopic pregnancy surgery
Objective To investigate the effect and safety of esmketamine on postoperative depression,anxiety and pain in patients undergoing laparoscopic ectopic pregnancy surgery.Methods A total of 152 patients who underwent laparoscopic ectopic pregnancy surgery in Zhongshan Xiaolan People's Hospital from January 2022 to July 2023 were selected and divided into an observation group and a control group according to the random number table method,with 76 cases in each group.The postoperative depression symptoms[self-rating depression scale(SDS)],anxiety symptoms[self-rating anxiety scale(SAS)],pain[visual analogue scale(VAS)]and safety were compared between the two groups.The influencing factors of postoperative depressive symptoms were analyzed.Results Three days after operation,the SDS scores of the two groups increased,but the SDS scores of the observation group were lower than those of the control group(P<0.05).The difference between the preoperative and postoperative SDS scores of the observation group was also lower than that of the control group(P<0.05).Three days after operation,SAS scores of the two groups decreased,and SAS scores of the observation group were lower than those of the control group(P<0.05).The difference between preoperative and postoperative SAS scores of the observation group was higher than that of the control group(P<0.05).The VAS scores of the observation group were lower tthose of the control group at 6 h,12 h and 24 h after operation(P<0.05).There was no statistically significant difference in the incidence of nausea,vomiting,dizziness,headache,chills and restlessness between the two groups(P>0.05).Linear regression analysis showed that Preoperative anxiety(β=0.334,95%CI=0.099-0.569)and 6h postoperative pain score(β=3.152,95%CI=1.280-5.024)were risk factors for postoperative depression in ectopic pregnancy patients(P<0.05).Esketamine regimen(β=-2.806,95%CI=-4.861--0.751)was a protective factor for postoperative depression in ectopic pregnancy patients(P<0.05).Conclusions The application of esmketamine in laparoscopic ectopic pregnancy can improve postoperative depressive symptoms without increasing adverse reactions,which is safe and reliable.Preoperative anxiety and pain 6 hours after surgery can increase the risk of postoperative depression,while esketamine can reduce anxiety and pain.
