Influencing factors and path analysis of psychological insulin resistancein patients with type 2 diabetes mellitus
Objective To explore the various factors affecting the psychological insulin resistance of patients with type 2 diabetes mellitus and their pathways of action. Methods A total of 204 patients with type 2 diabetes mellitus from a third-grade A-level hospital in Guangzhou were selected as the research subjects using convenient sampling.A self-designed general information questionnaire and an psychological insulin resistance scale were used for the survey. The data were analyzed using univariate analysis, multivariate analysis, multiple linear regression analysis, and path analysis. Results The total score of psychological insulin resistance was(59.99±8.56),with 104 cases(50.98%)scoring≥60,indicating the presence of psychological insulin resistance.Path analysis indicates that fear of addiction, concern over mastery of injection techniques, level of education, number of hypoglycemic episodes, worry about increasing financial burden, and oral hypoglycemic medications directly affect the attitude towards insulin treatment, with total effect values of 0.344,0.225,-0.222,0.194,0.153,and 0.119,respectively.Conclusions The psychological insulin resistance among patients with type 2 diabetes needs further improvement, which is influenced by multiple factors and pathways. This suggests that clinical workers should pay attention to the psychological state, educational level, and mastery of injection techniques of patients,in order to reduce psychological insulin resistance and improve the acceptance and usage rate of insulin treatment.
