摘要
目的 研究瘦素(Leptin)、白细胞介素-6(IL-6)玲慢性阻塞性肺疾病(COPD)稳定期患者营养不良发生的影响及其可能机制.方法 将60例COPD稳定期患者分为营养不良(Ⅰ)组及非营养不良(Ⅱ)组,以20例正常人为对照,均测量体质量指数(BMI)、理想体质量百分比(NW%),)肱三头肌皮皱厚度(TSF)、上臂中部臂围(MAC)、血清白蛋白(ALB)及淋巴计数(IYM),以放射免疫法和酶联免疫吸附试验测定血清瘦素与IL-6水平,分析瘦素与各项参数相关性.结果 (Ⅰ)三组(80例)的血清瘦素浓度与BMI,NW%,TSF,MAC呈显著正相关(P<0.001).(2)COPD营养不良组血清瘦素水平显著低于非营养不良与正常对照组,但非营养不良组与正常对照组,差异无显著性.(3)COPD营养不良组血清IL-6水平高于非营养不良组,且两组均高于正常对照组差异有显著性,但IL-6水平与血清瘦素水平无显著相关性.结论 血清瘦素与COPD患者营养参数相关,且COPD营养不良组血清IL-6水平升高,因此细胞因子--搜素联合作用可能是营养不良发生的原因之一.
Abstract
Objective To explore the significance of serum leptin and IL-6 in malnutrition of stable COPD patients. Methods To measure 60 cases stable COPD patients and 20 healthy controls body mass index (BMI), percent normal body weight (NW%), triceps skin-fold thickness (TSF), mid-upper arm circumference (MAC), serum albumin (ALB) and total lymphocytes count (LC). Leptin and IL-6 concentrations in and serum leptin and IL-6 concentrations were measured by radio immunoassay and ELISA. The correlation between leptin and nutritional parameters was analyzed. Results Serum leptin concentrations in 80 subjects were significantly correlated with BMI, NW%, TSF and MAC (P< 0.001). Serum leptin concentrations were significantly correlated with BMI in both malnourished patients with COPD and non malnourished patients (P<0.05). The leptin level was significantly lower in malnourished group than in non malnourished group (P<0.05). The difference between in serum leptin level and BMI between non malnourished group and healthy group were not significant. The serum IL-6 level was significantly higher in malnourished group than in non malnourished group and healthy group (P<0.05). The serum leptin levels didn't significantly correlate with IL-6 level in COPD penitents. Conclusion Serum leptin correlate with nutritional parameters in COPD patients and leptin, IL-6 may contribute to the malnutrition in stable COPD patients.
