Efficacy analysis of vascular resistance and uterine artery blood flow parameters in predicting adverse perinatal outcomes of hypertensive disorders during pregnancy
Objective To investigate the efficacy of maternal total vascular resistance(TVR)and uterine artery blood flow parameters in predicting perinatal adverse outcomes of hypertensive disorders during pregnancy(HDP).Methods 252 patients with hypertensive disorders in pregnancy admitted to Xuzhou Municipal Hospital affiliated to Xuzhou Medical University from April 2018 to January 2022,were retrospectively selected as the experimental group;252 healthy pregnant women examined during the same period were selected as the control group.Medical records and color Doppler ultrasound examination data were collected,and the parameters such as TVR,resistance index(RI),uterine artery pulsatility index(PI),peak systolic velocity/end-diastolic velocity(S/D)between the two groups of pregnant women were analyzed.The incidence of adverse perinatal outcomes in the experimental group was statistically analyzed.The experimental group was divided into the group with good outcomes and the group with adverse outcomes.The parameters such as the time from abnormal ultrasound detection to delivery,TVR,RI,PI,S/D,etc were compared between the two groups.The predictive efficacy of TVR,RI,PI,S/D and their combination for adverse perinatal outcomes in the experimental group was evaluated.Results The parameter values of TVR,RI,PI and S/D in the experimental group were significantly higher than those in the control group(t=34.638,21.604,19.451 and 18.021,respectively,P<0.05).The incidence of adverse perinatal outcomes in the experimental group was 15.48% .The parameter values of TVR,RI,PI and S/D in the adverse outcome group were significantly higher than those in the good outcome group(t=24.105,20.697,18.743 and 32.569,P<0.05),and the time from abnormal ultrasound detection to delivery was significantly shorter in the adverse outcome group than in the good outcome group(t=16.269,P<0.05).The area under the curve(AUC)value for the combined prediction of TVR,RI,PI and S/D was 0.961,higher than the prediction results of individual indicators.Conclusion The parameter values of TVR,RI,PI and S/D are higher in patients with early HDP,and the risk of adverse perinatal outcomes is also higher.Among those with adverse perinatal outcomes,the parameter values of TVR,RI,PI,and S/D are even higher.The combination of these four parameters can predict the risk of adverse perinatal outcomes.
