Analysis of MRI findings in osteonecrosis of the femoral head and relationship with prognosis
Objective To analyze the magnetic resonance imaging(MRI)findings and the relationship with prognosis in patients with osteonecrosis of the femoral head(ONFH).Methods Case data of 153 patients with ONFH who were clinically diagnosed between January 2019 and January 2022 were retrospectively analyzed.After admission,X-ray examination was performed before hip MRI examination,and X-ray and MRI findings of patients with ONFH were recorded.The patients were followed up for 12 months after surgery,and their prognosis was recorded.The MRI imaging signs of patients with different prognosis were compared and the relationship with prognosis was analyzed.Results Among the 153 patients with ONFH in this study,70 cases were detected by X-ray examination,including 110 femoral head necrosis[64(58.18%)femoral head cystic changes,39(35.45%)thickened and blurred bone trabeculae and 19(17.27%)increased bone mineral density].Two hundred seventeen cases of femoral head necrosis were diagnosed by MRI examination,comprising 69(97.18%)which were classified as Association Research Circulation Osseous(ARCO)stage Ⅰ,85(96.59%)as stage Ⅱ,44(100%)as stage Ⅲ and 19(100%)as stage Ⅳ,with a detection rate of 97.95%(217/222).The detection rate of MRI was higher than that of X-ray(P<0.05).The MRI images in the good prognosis group showed a smooth femoral head,high signal on T1-and T2-weighted images,and low signal on the femoral head.MRI examination in the poor prognosis group revealed patchy sclerosis,a"linear"femoral head with cystic degeneration,and articular surface collapse with degeneration.The proportions of femoral head morphological changes,spinal cord edema,joint effusion,T1W1 low signal,T2W1 high signal,and double line sign in the poor prognosis group were greater than those in the good prognosis group(P<0.05).Conclusion The specific MRI signs of patients with ONFH are femoral head collapse and joint effusion,which can effectively guide the disease staging of patients with ONFH and help guide the prognostic evaluation of patients,and it has certain clinical reference value.
