Clinical efficacy and safety analysis of denosumab in elderly women with osteoporotic intertrochanteric fractures after surgery
Objective To evaluate the efficacy and safety of denosumab in elderly women with osteoporotic intertrochanteric fractures who underwent proximal femoral nail anti-rotation(PFNA)internal fixation. Methods A prospective study was conducted involving 80 elderly women with osteoporotic intertrochanteric fractures who were treated at the Taixing People's Hospital from January to August 2023.The patients were randomly assigned to observation and control groups. Both groups underwent standard PFNA internal fixation surgery. The observation group received postoperative denosumab treatment and the control group receiving zoledronic acid. The fracture healing time, pain relief, hip function, bone mineral density, and incidence of adverse events were compared between the two groups. Results The fracture healing time was significantly shorter in the observation group than the control group(P<0.05).Both groups showed significant reductions in visual analog scale pain scores following treatment compared to baseline but the observation group had significantly lower visual analog scale scores than the control group 1,3,and 6 months post-treatment(P<0.05).Harris hip function scores improved significantly in both groups with greater improvements in the observation group(P<0.05).The bone mineral density of the non-fractured hip also improved more significantly in the observation group compared to the control group(P<0.05).Although the observation group had a lower incidence of adverse events, there was no statistically significant difference between the two groups(P>0.05).Conclusion Denosumab was shown to be effective in reducing pain and improving functional outcomes in elderly women with osteoporotic intertrochanteric fractures following PFNA internal fixation. Moreover, denosumab had superior clinical efficacy and a favorable safety profile.
