摘要

目的 探讨分阶段综合管理干预对妊娠期糖尿病患者血糖水平、分娩结局及并发症的影响。 方法 选取2020年4月至2022年4月复旦大学附属妇产科医院就诊的妊娠期糖尿病患者123例为研究对象,按照不同干预方式将患者分为对照组(常规护理干预,60例)和观察组(分阶段综合管理干预,63例),观察两组血糖控制水平、分娩结局以及新生儿和患者的并发症发生情况,并进行对比分析。 结果 干预后两组FBG、2hPG、HbA1c水平均明显降低,且观察组显著低于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率(12.70%)低于对照组(50.00%),差异有统计学意义(P<0.05)。观察组自然分娩率(88.89%)高于对照组(73.33%),差异有统计学意义(P<0.05)。观察组总发生率(7.94%)低于对照组(21.67%),差异有统计学意义(P<0.05)。 结论 对妊娠期糖尿病患者实施分阶段综合管理干预能有效控制患者血糖水平,提高自然分娩率,降低患者和新生儿并发症发生率,值得临床推广应用。 Objective To investigate the effects of staged comprehensive management intervention on blood glucose levels, delivery outcome and complications in patients with gestational diabetes mellitus. Methods A total of 123 patients with gestational diabetes mellitus who visited our hospital from April 2020 to April 2022 were enrolled in the study, and according to different intervention methods, the patients were divided into control group (routine nursing intervention, n=60) and observation group (staged comprehensive management intervention, n=63), the blood glucose control level, delivery outcome, and the incidence of complications of neonates and patients were observed, and were compared between the two groups. Results After the intervention, the levels of FBG, 2hPG, and HbA1c in the two groups were significantly reduced, and the observation group was significantly lower than the control group, the difference was statistically significant (P<0.05). he total incidence of complications in the observation group (12.70%) was lower than that in the control group (50.00%), and the difference was statistically significant(P<0.05). The natural delivery rate in the observation group (88.89%) was higher than that in the control group (73.33%), the difference was statistically significant (P<0.05). The total incidence of complications in the observation group (7.94%) was lower than that in the control group (21.67%), the difference was statistically significant (P<0.05). Conclusions Implementing staged comprehensive management intervention for gestational diabetes mellitus patients can effectively control blood sugar level, increase the rate of natural delivery, and reduce the incidence of complications in patients and newborns, which is worthy of clinical application.
