目的 探讨首发未用药精神分裂症患者CD4+CD25+Foxp3+调节性T细胞(Treg)及其相关炎性因子水平及临床意义.方法 选取2021年5月~2021年10月在青岛市精神卫生中心收治的41例首发精神分裂症患者为研究组,同期本院健康职工43例为对照组.采用流式细胞术检测两组外周血CD4+CD25+Foxp3+Treg细胞百分比及转化生长因子-β(TGF-β)、叉头状转录因子p3(Foxp3)mRNA相对表达水平;酶联免疫吸附法(ELISA)检测血清白细胞介素-6(IL-6)和白细胞介素-35(IL-35)水平;分析 CD4+CD25+Foxp3+Treg 细胞百分比与 TGF-β、Foxp3、IL-6 和 IL-35 表达的相关性,及其与阳性与阴性症状量表(PANSS)的关系.结果 首发精神分裂症组患者外周血CD4+CD25+Foxp3+Treg细胞百分比、Foxp3mRNA及血清IL-35水平明显低于对照组,而外周血TGF-βmRNA表达及IL-6水平则明显高于对照组,差异有统计学意义(均为P<0.001).精神分裂症患者外周血CD4+CD25+Foxp3+Treg细胞百分比与外周血Foxp3 mRNA水平(r=0.913)和血清IL-6水平(r=0.771)呈正相关(P<0.001),与外周血TGF-β mRNA水平(r=-0.952)、IL-35(r=-0.908)呈负相关(P<0.001).精神分裂症患者外周血CD4+CD25+Foxp3+Treg细胞百分比与PANSS总分及各维度均呈负相关(P<0.001).结论 精神分裂症患者外周血CD4+CD25+Foxp3+Treg细胞及相关炎性因子的表达有异常,且与病情的严重程度有一定的相关性,可能与患者体内免疫抑制机制受损有关.外周CD4+CD25+Foxp3+Treg细胞调控的细胞免疫异常可能是精神分裂症发病的神经免疫学机制之一.
Levels and clinical significance of Treg cells and their associated inflammatory factors in patients with first episode schizophrenia
Objective To investigate the levels and clinical significance of CD4+CD25+Foxp3+regulatory T cells(Treg)and the related inflammatory factors in patients with schizophrenia without first medication.Methods 41 patients with first-stage schizophrenia admitted to Qingdao Mental Health Center from May 2021 to October 2021 were selected as the study group,and 43 healthy workers in our hospital were selected as the control group during the same period.The percentage of CD4+CD25+Foxp3+Treg cells and the mRNA expression of transforming growth factor-β(TGF-β)and fork-head transcription factor p3(Foxp3)in peripheral blood of the two groups were detected by flow cytometry.Serum levels of interleukin-6(IL-6)and interleukin-35(IL-35)were determined by enzyme-linked im-munosorbent assay(ELISA).The correlation between the percentage of CD4+CD25+Foxp3+Treg cells and TGF-β,Foxp3,IL-6 and IL-351evelswere analyzed,as well as the correlation with the positive and negative symptom scale(PANSS).Results The percentage of CD4+CD25+Foxp3+Treg cells,the expression of FoxP3 mRNA and the level of serum IL-35 in peripheral blood of patients with first-onset schizophrenia were significantly lower than the control group,while the expression of TGF-β mRNA and the level of IL-6 in peripheral blood were significantly higher than the control group,with statistical significance(P<0.001).The percentage of CD4+CD25+Foxp3+Treg cells in periph-eral blood of patients with schizophrenia was positively correlated with peripheral blood Foxp3 mRNA level(r=0.913)and serum IL-6 level(r=0.771)(P<0.001).And TGF-β mRNA level(r=-0.952),IL-35(r=-0.908)in peripheral blood were negatively correlated(P<0.001).The percentage of CD4+CD25+Foxp3+Treg cells in peripheral blood of patients with schizophrenia was negatively correlated with the total score of PANSS and all dimensions(P<0.001).Conclusion The expression of CD4+CD25+Foxp3+Treg cells and related inflammatory factors in peripheral blood of patients with schizophrenia,and there is a certain correlation with the severity of the disease,which may be re-lated to the weakened immunosuppressive function in patients.Abnormal cellular immunity regulated by peripheral CD4+CD25+FOXP3+Treg cells is one of the neuroimmunological mechanisms of schizophrenia.
