摘要
目的 探讨社会心理干预(social psychic intervention,SPI)和睡眠脑电QUISI辅助检测指导对精神分裂症患者睡眠的影响.方法 招募148例精神分裂症住院患者,应用随机数字表法,分成试验组和临床组各74例,试验组采用SPI("二干一护一康"新方案)和临床组采用普通支持,疗效评估者盲法,QUISI对精神分裂症睡眠改善进行监测.结果 (1)采用SPI后,试验组的精神症状自评量表的减分显著高于临床组(P＜0.05).试验组的简明精神病评定量表减分显著高于临床组;试验组自知力与社会功能缺陷测定治疗依从性问卷增分高于临床组(P＜0.05～0.01).(2)试验组随着康复,QUISI检测中的主要指标醒觉时间、觉睡比和睡眠效率也随着改善(睡眠效率干预前85.1± 8.8％,干预3个月95.4±3.1％,t=5.1001,P＜0.001).结论 SPI疏导能够有效缓解精神分裂症患者的负面情绪和改善睡眠,进一步促进患者临床症状的恢复.在精神科康复中有推广应用价值.
Abstract
Objective To explore the effect of social psychological intervention(social psychic intervention,SPI)nursing and sleep EEG QUISI diagnosis and treatment on the sleep of schizophrenic patients.Methods 148 schizophrenic inpatients were recruited and divided into test group and control group with 74 cases each by random num-ber table.The test group was given SPI care and the control group was given general support.The efficacy evaluator was blind.QUISI monitored the sleep improvement of schizophrenia.Results(1)After using SPI,the scores of SCL-90 in the test group were significantly higher than those in the control group(P＜0.05).After the use of SPI,the scores of BPRS in the test group were significantly higher than those in the control group.The scores of insights and social dys-function test in the test group were higher than those in the control group(P＜0.05～0.01).(2)With the rehabilitation of the test group,the main indicators of QUISI detection,such as awakening time,sleep to sleep ratio and sleep ef-ficiency,also improved(sleep efficiency 85.1±8.8％before intervention,95.4±3.1％after three months of interven-tion,t=5.100 1,P＜0.001).Conclusion SPI intervention for schizophrenia patients is beneficial to the rehabilitation of symptoms and social functions of patients.Application in the rehabilitation and nursing of psychiatric department.
