目的 探讨慢性阻塞性肺疾病(慢阻肺)患者负性情绪与疲劳程度、慢阻肺评估测试(COPD Assessment test TM,CAT)关系.方法 纳入界首市人民医院2021年2月至2023年2月共90例慢阻肺患者,采用汉密尔顿抑郁/焦虑量表(Hamilton depression/anxiety scale,HAMD/HAMA)进行负性情绪评估,并根据评估结果设为负性情绪组以及无负性情绪组.比较两组患者一般资料、Chalder疲劳量表、慢阻肺评估测试(COPD Assessment test TM,CAT),分析HAMD评分、HAMA评分与疲劳评分、CAT评分的相关性.结果 90例慢阻肺患者检出焦虑情绪7例,抑郁5例,焦虑合并抑郁18例,负性情绪组30例,无负性情绪组60例;两组性别、年龄、受教育年限、病程、体质指数差异无统计学意义(P>0.05);负性情绪组体力疲劳、脑力疲劳评分显著高于无负性情绪组,CAT评分显著高于无负性情绪组(P<0.05);负性情绪组患者HAMD评分、HAMA评分与疲劳评分、CAT评分呈正相关(P<0.05).结论 慢阻肺患者负性情绪检出率较高,且焦虑、抑郁情绪可能增加患者疲劳程度,加重慢阻肺病情.
The relationship between negative emotions and fatigue level,CAT in patients with chronic obstructive pulmonary disease
Objective To explore the relationship between negative emotions and fatigue levels,as well as COPD Assessment Test TM(CAT)in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 90 patients with chronic obstructive pulmonary disease in Jieshou City People's Hospital from February 2021 to Feb-ruary 2023 were included.The Hamilton Depression/Anxiety Scale(HAMD/HAMA)were used to evaluate the nega-tive emotion assessment,and based on the evaluation results,They were set as the negative emotion group and the non negative emotion group.The general information,chalder fatigue scale,and COPD assessment test(CAT)between two groups of patients were compared.The correlation between HAMD score,HAMA score,fatigue score,CAT score were analyzed.Results Among the 90 patients with chronic obstructive pulmonary disease,7 had anxiety,5 had de-pression,18 had anxiety combined with depression,30 had negative emotions,and 60 had no negative emotions.There was no statistically significant difference in gender,age,years of education,course of illness,and body mass index between the two groups(P>0.05).The Physical fatigue,mental fatigue scores of the negative emotion group were sig-nificantly 1 higher than those of the non negative emotion group,and the CAT scores were significantly higher than those of the non negative emotion group(P<0.05).The HAMD score and HAMA score of patients with positively emotions were negatively correlated with fatigue score and CAT score(P<0.05).Conclusion The detection rate of negative emotions in patients with chronic obstructive pulmonary disease is relatively high,and anxiety and depression may in-crease the patient's fatigue level and worsen the condition of chronic obstructive pulmonary disease.
