成年男性精神分裂症杏仁核神经代谢物与PANSS评分的相关性

目的 基于氢质子磁共振波谱(Hydrogen Proton Magnetic Resonance Spectroscopy,1H-MRS)探究成年男性精神分裂症患者杏仁核神经代谢物水平与阳性及阴性症状精神症状评定量表(Positive and Negative Syndrome Scale,PANSS)评分的相关性。方法 纳入 2021年4月~2023年6月湖南省第二人民医院精神科收治的59例成年男性精神分裂症患者作为患者观察组,另纳入18例健康体检者作为健康对照组。所有对象均行PANSS量表评分、MRI以及杏仁核区1H-MRS检测。收集纳入者左右侧杏仁核N-乙酰基天门冬氨酸(N-acetylaspartate,NAA)、肌酸(Creatine,Cr)和胆碱(Choline,Cho)水平,比较两组基线资料,比较两组Cho/Cr、NAA/Cho、NAA/(Cho+Cr)比值的差异,分析患者组杏仁核神经代谢物指标与PANSS评分的相关性。结果 男性精神分裂症患者左侧杏仁核的NAA/(Cho+Cr)、NAA/Cr、NAA/Cho、Cho/Cr比值均高于健康对照组,差异具有统计学意义(P<0。05)。Pearson相关性结果显示,患者组左侧杏仁核NAA/(Cho+Cr)、NAA/Cr、NAA/Cho与阴性症状评分呈负相关,相关性有统计学意义(P<0。05)。结论 基于1H-MRS发现男性精神分裂症患者左侧杏仁核区神经代谢物水平异常增高,可能与患者PANSS评估中阴性症状相关。
The alteration of neurotransmitters in amygdala and its association with PANSS score in male adults with schizophrenia
Objective Based on hydrogen proton magnetic resonance spectroscopy(1H-MRS),this study ex-plored the correlation between neurotransmitters in the amygdala and the scores of the Positive and Negative Symptom Scale(PANSS)in male adults with schizophrenia.Methods A total of 59 adult male patients with schizophrenia ad-mitted to the Department of Psychiatry of Hunan Second People's Hospital from April 2021 to June 2023 were included as the patient observation group,and 18 healthy subjects were included as the healthy control group.All patients un-derwent PANSS scoring,Magnetic Resonance Imaging,and 1H-MRS of the amygdala region.The levels of N-acety-laspartate(NAA),Creatine(Cr),and Choline(Cho)in the left and right amygdala of all participants were collected.Compare baseline data between the two groups,compare the differences in Cho/Cr,NAA/Cho,and NAA/(Cho+Cr)ratios between the two groups,and analyze the correlation between amygdala nerve metabolite indicators and PANSS scores in the patient group.Results The NAA/(Cho+Cr),NAA/Cr,NAA/Cho,Cho/Cr ratios in the left amygdala of male patients with schizophrenia were higher than those in the healthy control group,and the difference was statisti-cally significant(P<0.05).Pearson correlation analysis showed that the NAA/(Cho+Cr),NAA/Cr,NAA/Cho in the left amygdala of the patient group were negatively correlated with negative symptom scores,and the correlation was statistically significant(P<0.05).Conclusion Based on 1H-MRS,it was found that the NAA/(Cho+Cr),NAA/Cr,NAA/Cho,and Cho/Cr ratios in the left amygdala of male patients with schizophrenia were abnormally increased,which may be related to negative symptoms in PANSS score.
