计算机认知矫正治疗对男性精神分裂症患者认知功能的影响-基于C-BCT的评估

目的 探讨计算机认知矫正治疗(Computerzied Cognitive Remediation Therapy,CCRT)对男性精神分裂症患者认知功能的影响。方法 纳入87名稳定期男性精神分裂症患者,随机分为观察组(CCRT+抗精神病药物治疗,45名)、对照组(单一抗精神病治疗,42名),基线时以及康复训练结束时采用中国简版神经认知成套测验(The Chinese Brief Cognitive Test,C-BCT)评估入组患者的认知功能,并使用采用阳性和阴性症状量表(positive and negative syndrome scale,PANSS)评估患者病情。结果 对照组与观察组基线时的一般人口学信息、PANSS总分评分差异无统计学意义(P>0。05),观察组患者经CCRT康复训练后在卫星探索、速度匹配、颜色stroop、迷途小鱼上前后变化差异有统计学意义(P<0。05),双重视觉记忆、速度分类差异无统计学意义(P>0。05),观察组经CCRT康复训练后认知功能与对照组比较,其中总T分、连线T分、持续操作T分差异具有统计学意义(P<0。05),而数字广度T分、符号编码T分差异无统计学意义(P>0。05)。结论 CCRT康复训练联合抗精神病药物治疗较单一抗精神病药物治疗能更有效改善男性精神分裂症患者的认知功能。
Effects of computerized cognitive remediation therapy on cognitive functioning in male schizophrenic patients-an assessment based on C-BCT
Objective To explore the effects of Computerzied Cognitive Remediation Therapy(CCRT)on cog-nitive function in male schizophrenic patients.Methods Eighty-seven stable male schizophrenic patients were ran-domly divided into an observation group(CCRT+antipsychotic medication,45 patients)and a control group(single antipsychotic treatment,42 patients),and their cognitive functioning was assessed at baseline and at the end of the rehabilitation training using the Chinese Brief Cognitive Test(C-BCT).At baseline and at the end of the rehabilitation training,the cognitive function of the enrolled patients was assessed by The Chinese Brief Cognitive Test(C-BCT),and the condition of the enrolled patients was assessed by the positive and negative syndrome scale(PANSS).Results The differences in general demographic information and PANSS total score scores between the control group and the observation group at baseline were not statistically significant(P>0.05),the differences in before and after changes in satellite exploration,speed matching,color stroop,and lost minnow between the patients in the observation group af-ter CCRT rehabilitation training were statistically significant(P<0.05),and the differences in dual visual memory and speed categorization were not statistically significant(P>0.05),and the differences in the patients'cognitive function-ing between the observation group and the control group after CCRT rehabilitation training were statistically significant(P>0.05),the cognitive function of the observation group after CCRT rehabilitation training compared with the control group,in which the differences in total T-score,connectivity T-score,and sustained operation T-score were statisti-cally significant(P<0.05),while the differences in digit breadth T-score,and symbol encoding T-score were not sta-tistically significant(P>0.05).Conclusion CCRT rehabilitation training combined with antipsychotic medication can be more effective than a single antipsychotic medication to improve the cognitive function of male schizophrenic patients.
